进展期胃癌术后营养状态对化疗影响的临床观察

目的：观察进展期胃癌术后不同营养状态患者化疗期间的临床状况差异性，探讨营养治疗在营养不良患者化疗期间的临床意义。方法运用营养风险筛查（NRS2002）评价标准，选取进展期胃癌术后NRS＞3分的80例患者，随机分为实验组和对照组，每组各40例。实验组患者进行肠外营养支持，对照组除不予肠外营养支持外其余同实验组。比较两组患者的营养生化指标、化疗毒副反应。结果实验组和对照组在营养生化指标、化疗毒副反应评分差异上有统计学意义（P＜0．05）。结论营养不良状态是影响胃癌术后化疗的不利因素，降低化疗耐受力、生活质量，影响胃癌术后化疗的临床转归，营养支持治疗使营养不良患者化疗期间临床受益。

Clinical observation about the impact of postoperative nutritional status on chemotherapy for ad- vanced gastric carcinoma

Objective To observe the clinical differences of postoperative nutritional status inchemotherapy patients with advanced gastric carcinoma and discuss the significance of nutritional therapy.Method According to the Nutritional Risk Screening 2002（NRS 2002）,80 patients（NRS score 〉3）wererandomly divided into the experimental group（n =40）and control group（n =40）.Patients in the experi mental group received additional parenteral nutrition.Patients in the control group received the same treat ment except for the parenteral nutrition.Nutritional biochemistry indexes,toxicities and side reactions ofthe chemotherapy were compared between the groups.Results There were significant differences in nutri tional biochemistry indexes,toxicities and side reactions of the chemotherapy between the groups（P 〈0.05）.Conclusion Poor nutrition status is a negative factor of postoperative chemotherapy for patientswith gastric carcinoma.It will reduce chemotherapy tolerance and quality of life,which has negative effectson the clinical outcomes of postoperative chemotherapy.Nutrition support will benefit those patients whohad malnutrition during the time of chemotherapy.
