肝硬化患者行腹腔镜胆囊切除术32例临床分析

目的：总结肝硬化患者行腹腔镜胆囊切除术（laparoscopic cholecystectomy,LC）的经验、手术技巧及可能存在的风险。方法：回顾分析我院2004年1月至2007年12月为32例Child A、B级肝硬化患者行LC的临床资料。结果：30例患者均顺利完成LC,2例因术中止血困难而中转开腹。无死亡病例及肝功能衰竭等严重并发症发生。结论：肝硬化患者行LC难度较大,风险较高,术中分离困难,但只要严格掌握手术适应证,注意围手术期处理,有熟练的腹腔镜手术经验,肝硬化患者行LC是安全可行的。

Laparoscopic cholecystectomy in patients with hepatic cirrhosis:a report of 32 cases

Objective:To discuss the clinical experience,operation skill and the potential risk of laparoscopic cholecystectomy(LC)in patients with cirrhosis.Methods:The clinical data of 32 cases with cirrhosis undergoing LC from Jan.2004 to Dec.2007 were retrospectively analyzed.Results:Thirty cases with laparoscopic cholecystectomy were succeeded,2 cases were converted to open laparotomy because of hemorrhage in gallbladder triangle.There were no deaths and complications as hepatic failure.Conclusions:LC is safe and effective as treatment on patients complicated with hepatic cirrhosis.
