胆囊结石嵌顿的腹腔镜处理

目的 :总结胆囊结石嵌顿腹腔镜胆囊切除术的治疗经验。方法 :回顾分析胆囊颈部结石嵌顿85例的腹腔镜处理。结果 :中转开腹 3例 ,1例胆总管横断伤中转开腹行胆总管对端吻合“T”管引流 ,术后 6月拔管治愈。 1例胆囊动脉分支出血中转开腹止血 ,1例术中快速病理切片证实为胆囊癌行胆囊癌根治术。全组无死亡病例。结论 :经过充分术前准备 ,合理选择中转开腹时机 ,术者熟练的操作 ,胆囊结石嵌顿行腹腔镜胆囊切除术是安全可行的

Laparoscopic management for calculus incarcerated cholecystitis

Objective:To summarize the experience of treatment for calculus incarcerated cholecystitis by laparoscopic cholecystectomy(LC).Methods:85 cases with calculus incarcerated cholecystitis by LC were retrospectively analysed. Results:There was no death in all patients.Three cases were converted to laparotomy.One case with common bile duct converting injury was cured with converting open operation by choledochocholedochostomy with T tube drainage and relieved six months postoperatively.One case of cystic artery branches bleeding was halted by converting open operation,one case was confirmed gallbladder cancer by intraoperative frozen section and done radical operations.Conclusions:To be fully prepared before operation,LC is a safe and feasible means for calculus incarcerated cholecystitis.
