体外循环与非体外循环冠状动脉旁路移植术病人住院费用的对比研究

目的对比研究体外循环冠状动脉旁路移植术（CCABG）与非体外循环冠状动脉旁路移植术（OPCAB）病人住院费用的差别。方法将679例行冠状动脉旁路移植术（CABG）病人分为CCABG组与OPCAB组，对比两组住院费用及各项分类住院费用的差别，包括西药费、化验费、检查费、治疗费、手术费、麻醉费、血费、材料费、护理费、膳食费及其他费用。结果CCABG组合计住院费用高于OPCAB组，但差异无统计学意义；在分类费用中，CCABG组的西药费、手术费、血费、床位费及护理费高于OPCAB组，而OPCAB组材料费高于CCABG组，差异有统计学意义；两组间化验费、检查费、治疗费、麻醉费、膳食费及其他费用差异无统计学意义。结论OPCAB组合计住院费用与CCABG组相仿，而OPCAB组的材料费用显著高于CCABG组。

Economic outcome of coronary artery bypass surgery: a comparative study between on-pump and off-pump surgery

Objective To compare the economic outcome between on-pump and off-pump coronary artery bypass grafting (CABG). Methods Six hundred seventy-nine patients were divided into on-pump and off-pump groups. Twelve variables with respect to costs were obtained for each group during hospital stay. Results There was no significant difference in total mean cost between two groups. For off-pump patient, the costs of medication, operation, blood transfusion, bed occupancy, and nursery were significantly less than those of on-pump CABG surgery was (P<0.05), which reflected the cost advantage of OPCAB over CCABG. The costs of operating materials in off-pump group were higher than those of on-pump group were (P<0.05). The other costs regarding blood tests, examination, anesthesia, and nutrition were not significantly different between the two groups (P>0.05). Conclusion Costs of OPCAB are not significantly lower than on-pump CABG due to higher material costs.
