老年人脊髓型颈椎病的特点及手术治疗

摘    要:目的 探讨老年人脊髓型颈椎病的临床特点及手术方法。方法 对26例60-81岁老年人脊髓型颈椎病患者行颈前路开槽式减压,24例取自体髂骨移植,并行带锁钢板内固定,2例用Cage内充填减压椎体之松质骨植入。结果 23例获随访,平均随访时间为24.6个月,植骨融合率为100%,术后椎间高度及颈椎生理曲度维持良好,临床疗效优良率73%。结论 老年患者由于全身多器官功能退化,颈部骨质退变,临床表现复杂,多节段发病,诊断及治疗都存在一定的难度,只要选择适当,高龄并非手术禁忌症。

Abstract:Objective:To investigate the characteristics and operative treatment of the cervical spondylomyelopathy in the elderly Methods:26 cases(aged from 60 to 81 years old) of the cervical spondylo myelopathy were treated with slot decompression with anterior approach;24 of them were treated with fusion using autogenous iliac bone graft and self lock plate internal fixation;2 were treated with Cage implant with vertebral cancellous bone. Results:23 cases were followed up with a mean preiod of 24.6 months.The fusion rate was 100% and the height of disc space,the lordosis of cervical spine were restored well after operation.The rate of excellent and clinical effect was 73%. Conclusion:With the degeneration of the cervical spine and multiple organs in the elderly as well as the complexed clinical presentation with multiple segmental myelopathy,both the diagnosis and treatment are tough problems;however,the cases were selected carefully,the age of the patients in itself should not be considered as a surgical contraindication.
