下颈椎骨折脱位并脊髓损伤的前路手术治疗

目的探讨前路手术在治疗下颈椎骨折脱位并脊髓损伤中的价值。方法168例下颈椎骨折脱位并脊髓损伤患者均在全麻下行颈前路减压、复位、钛网或自体髂骨植骨及颈椎带锁钢板固定。结果完全复位151例,复位90%以上17例。平均随访80.7个月,颈椎椎间高度和生理曲度维持良好,无钢板螺钉并发症。140例脊髓损伤者神经功能获改善。结论下颈椎骨折脱位并脊髓损伤选择前路手术治疗可获得满意的疗效。

Surgical treatment of lower cervical fractures and dislocations combined with spinal cord injuries by anterior approach

Objective To discuss the value of surgical treatment for lower cervical fractures and dislocations combined with spinal cord injury by anterior approach. Methods The clinical data of 168 patients with lower cervical spine fractures and dislocations were retrospectively analyzed. All the cases were operated in general anaesthesia by anterior decompression, reduction, auto-iliac graft and cervical spine locking plate fixation. Results Complete reduction was got in 151 cases, and 17 cases obtained more than 90% reduction. The follow-up time was 80.6 months in average. The normal intervertebral height and physiological curvature were maintained, and there were no plates or screws complications. After operation, 140 cases with neurological defect got improved. Conclusion For severe lower cervical fractures and dislocations, the satisfied curative effect can be obtained by choosing anterior approach surgery.
