肝细胞癌门静脉主干癌栓uPA和uPAR基因表达

目的：探讨uPA,uPAR基因表达和肝癌转移，肝细胞癌门静脉主干癌栓（Tumor thrombosid of the main trunk of the portal vein,PVTT)形成的关系。方法：采用原位杂交等技术检测uPA,uPAR表达。结果：癌栓（A1组）和其原发癌（A2组）uPA,uPAR mRNA阳性表达率均高于无转移肝癌（B组）（P均＜0．01），A1组表达强度均高于A2组（P＜0．01），A2组小血管内可见癌细胞群落，A1组多见，B组无此现象，A1组、A2组uPAR和uPA蛋白质阳性表达率均高于B组（P均＜0．01），A1组表达强度均高于A2组（P＜0．01），uPA,uPAR mRNA和其蛋白质表达存在良好相关性（P均＜0．01），结论：uPA,uPAR在肝癌的侵袭和转移中起重要作用。其过度表达是PVTT形成和肝癌转移的重要因素。

EXPRESSION OF UROKINASE-TYPE PLASMINOGEN ACTIVATOR AND UROKINASE-TYPE PLASMINOGEN ACTIVATOR RECEPTOR IN THE PVTT OF HCC

Objective To determine the expression of serine proteases in PVTT of HCC,the association of these enzymes with the invasive and metastatic properties of HCC and the formation of PVTT.Methods Immunohistochemistry and in situ hybridization were performed to study the distribution of a urokinase type plasminogen activator(u PA) and it's receptor(u PAR).The intensity of u PA,u PAR mRNA and protein were evaluated using a computer image analyzer cell analysis system.Results uPA mRNA and uPAR mRNA were expressed heterogeneously in cancer cells,and restricted expression was found in stromal cells,especially fibroblasts that were located in the immediate proximity of the cancerous cells,higher in edge of tumor and around of small blodd vessel in tumor than the other.In 20 HCC without PVTT and metastasis (group B),the expression positive rates of uPA mRNA and uPAR mRNA were 40% and 45%.In HCC with PVTT (group A2) and PVTT )(group A1).uPA mRNA and uPAR mRNA expression were detected in all specimens.the expression rated of uPA mRNA and uPAR mRNA in group A2 and A1 were all higher than in group B(P<0.01).The intensity of uPA mRNA in group A1 was higher than group A2(P<0.01),same as uPAR mRNA,Venous invasion was detected in the group A2 ,more tumor cell colony detected in the group A1.The distribution uPAR/uPA mRNA and protein were usually very similar in the group B,A2,A1.There was a statistically significant correlation between the intensity of uPAE,uPAmRNA and uPAR,uPA proteinexpression.Conclusion The expression of uPA/uPAR may play a important role in the invasion and metastasis of HCC,Over expression of uPA/uPAR may be a critical factor in formation of PVTT and metastasis of HCC.
