强直性脊柱炎髋屈曲挛缩畸形的全髋关节置换术

目的：观察对于强直性脊柱炎髋屈曲挛缩畸形实施全髋关节置换术的临床效果。方法：自1998年7月至2005年9月对18例（36髋）强直性脊柱炎髋屈曲挛缩畸形的患者行全髋关节置换术，术后平均随访3．2年，对手术前后疼痛、活动度、畸形矫正等进行了对比。结果：术后除6侧关节轻度疼痛外，其余关节均无疼痛。关节活动度明显增加，Harris评分由术前平均31分改善为76分（配对t检验，P〈0．1）。结论：全髋关节置换术是治疗严重的强直性脊柱炎髋屈曲挛缩畸形的一种有效的方法。

Total Hip Replacement for Hip Flexion Deformity of Ankylosing Spondylitis

Objective：To observe the effect of the total hip replacement in patients with hip flexion deformity of ankylosing spondylitis （AS）. Methods： Thirty-six hips of 18 patients with hip deformity of AS underwent total hip replacement. The mean duration of follow-up was 3.2 years. The pre- and post-operative comparative study on pain, range of motion （ROM）, correction of deformity and Harris score were carried out. Results： Pain relief ob- tained in all but 6 hips. Motion of joint was improved significantly, preoperative Harris score was mean 31 and improved to 76 postoperatively （P〈0.1）. Conclusion： Total hip replacement is an effective method for patients with the flexion deformity of AS .
