椎弓根钉-椎板钩系统治疗青少年腰椎峡部裂的临床疗效

目的探讨椎弓根钉-椎板钩系统在治疗青少年腰椎峡部裂时的临床疗效。方法从1999-02-2011-02,应用椎弓根钉-椎板钩节段内固定及植骨修复重建青少年腰椎峡部裂22例。保守治疗至少6个月无效,术前腰痛VAS评分8.1分,术中彻底清除峡部裂之间的纤维组织、瘢痕及增生组织,切除裂部骨端的硬化骨,至断面出现渗血为止,取自体髂骨条植骨,安放椎弓根钉-椎板钩系统。结果所有患者术后随访9～48个月,平均24个月。术后恢复满意,无内固定断裂、神经损伤等并发症,术后腰痛VAS评分降到2.1分,所有病例均获得骨性愈合。结论椎弓根钉-椎板钩系统及植骨修复重建青少年腰椎峡部裂效果确实,值得推广应用。

Clinical outcome of pedicle screw-lamina hook system for the surgical treatment of adolencent lumbar spondylolysis

Objective To evaluate the clinical results of pedicle screw-lamina hook system in treating the adolencent lumbar spondylolysis.Methods 22 patients with lumbar spondylolysis suffered from severe back pain for more than six months with failure of conservative treatment.All patients were treated with a pedicle screw-lamina hook system between February 1999 and February 2011.The average VAS before the operation was 8.1.During the operation,any fibrous tissues in the defects were cleared off,The sclerotic bone margins were curetted until bleeding.the defect is filled with cancellous bone graft harvested from the iliac crest.The pedicle screw-lamina hook system was inserted.Results All patients followed-up for average 12 months(9~48months) got significant back pain relief.The postoperative VAS was 2.1.The bony healing were comfirmed in all the cases.Conclusion The simple spondylolysis in adolencence can be treated with the pedicle screw-lamina hook system with good clinical outcomes.
