先天性肌性斜颈的手术治疗(附72例分析)

先天性肌性斜颈是常见的颈部畸形。临床多采用手术治疗。本文认为,手术最佳年龄2～4岁,主张5岁以前采用胸、锁二头切断加挛缩病肌切除;5～8岁行胸、锁、乳突三头切断加挛缩病肌切除;8岁以上用胸锁乳突肌全部切除或扩大全切,石膏帽背甲外固定,提倡术后6～7天拆线后一次完成。强调术前术后进行认真持久的胸锁乳突肌按摩、热敷和牵伸训练。

Operation For Ccogenital Muscular Torticollis

Congential muscular torticollis is a common neck malformation and it generally treated with surgery. We suggest that the optimum age for operation is 2 to 4 years old. For the mild cases under 5 years old the clavicular and sternal heads of the muscle were amputated and part of the muscle was excised. For the cases aged from 5 to 8 the all heads of the muscle and most of the muscle were resectted and removal of the all heads and all the muscle were carried out in the cases aged over 8 years. We recommend that external fixation of the spinal brace and plaster cap should be completed at a time after taking out the suture at the sixth or seventh day postoperatively. We emphasize that continuous massage, hot compress and traction of stern-oclidomastoid muscle should be performed before and after operation.
