康复治疗对髋关节置换患者术后关节功能及生活质量的影响

目的：观察康复训练对人工全髋关节置换患者术后功能恢复及生活质量的影响。方法选取2010年5月至2014年6月于我院行全髋关节置换术且同意配合调查的120例患者，采用随机数字表法分为两组，常规护理组（60例）给予常规护理治疗，康复治疗组（60例）在常规护理基础上进行有计划、分阶段的系统康复治疗。分别于术后1周、1个月、3个月采用髋关节Harris评分系统评价患者髋关节功能，术后1周、3个月采用世界卫生组织生活质量测定量表简化版（WHOQOL?BREF）评估患者术后的生活质量。结果两组患者的髋关节Harris评分、WHOQOL?BREF评分均逐步改善；康复治疗组患者术后1、3个月时的髋关节Harris评分和术后3个月时的WHOQOL?BREF评分均显著优于常规护理组（均P＜0.05）。结论全髋关节置换患者术后辅以分阶段系统的康复训练及心理治疗，有助于患者肢体功能的恢复及生活质量的提高，具有重要的临床及社会意义。

Effect of rehabilitation treatment on quality of life of patients with hip arthroplasty

Objective To observe the effect of rehabilitation therapy on functional recovery and the quality of life of patients after total hip arthroplasty (THA). Methods Cases of 120 patients after THA were divided into two groups randomly. The control group (60 cases) was treated with the conventional nurs?ing, and the rehabilitation group (60 cases) was given planned and phased system rehabilitation. The Harris hip joint function Scale was used to evaluate the function of hip joint at 1st week, 1st month and 3rd month after THA, and the WHOQOL?BREF was used to evaluate the quality of life at 1st week and 3rd month after THA. Results The Harris score and WHOQOL?BREF score were improved gradually after THA. The Harris score at 1st month and 3rd month after THA, and the WHOQOL?BREF score at 3rd month in the rehabilitation group were significantly higher than those in the control group (P<0.05 for all). Conclusion Rehabilitation therapy and psychological therapy after THA could further improve the function of hip joint and the living quality of the patients, and had important clinical and social significance.
