腕管综合征术前肌电指标与术后早期疗效的相关性分析

目的探讨腕管综合征术前肌电指标与术后早期疗效的相关性.方法将50例患者术前肌电检测值分为轻、中、重3组并进行评分及分组.术后3个月复查,疗效按Kelly评定方法评分.将术前肌电检测值与术后疗效进行相关性分析.结果正中神经感觉传导速度轻度组术后拇指到腕较术前增加21.5%,中指到腕增加16.3%.中度组术后拇至腕较术前增加17.7%,中指到腕增加21.9%.重度组术后正中神经末端CMAP潜伏期较术前加快28.6%.3组肌电指标术前、后相比差异均有显著性意义(P＜0.05).术前肌电指标组合分组与术后早期疗效评分之间做Spearman等级相关分析,两者呈显著负相关(rs'=-0.6172,P＜0.001).结论术前肌电指标组合分组可以作为术后早期疗效的预测指标.

Analysis of correlations between early outcomes of carpal tunnel release and pre operative electromyogramic results

Objective To analyze the correlations between early outcomes of carpal tunnel release and the results of preoperative electromyogramic examination.Methods 50 patients were divided into three groups (mild, moderate, and severe) according to electromyogramic results. The early treatment outcomes at postoperative month 3 were evaluated according to Kelly's methods. The correlations between postoperative outcome and preoperative electromyogramic results were made by Spearman's rank analysis. Results In mild group, sensory nerve conduction velocity of the median nerve increased 21.5 % and 16.3 % postoperatively from thumb and middle finger to wrist ,respectively,and 17.7 % and 21.9 % were obtained in moderate group respectively. In severe group, the latency of CMAP in the median nerve decreased 28.6 % postoperatively. There was statistically significant difference in 3 groups between preoperative and postoperative one. Spearman's rank correlation analysis was done between preoperative electromyogramic grading and postoperative early outcomes, with a negative result. Conclusions Preoperative electromyogramic results might be a predictor of early treatment outcome in carpal tunnel release.
