Chemerin在合并2型糖尿病口腔鳞癌患者肿瘤中的表达及临床意义

摘    要:目的: 探讨chemerin在合并2型糖尿病口腔鳞癌(T2DM-OSCC)患者肿瘤组织中的表达及临床意义,分析其与临床病理参数及预后的关系,为T2DM-OSCC患者的治疗提供理论依据。方法: 收集2009年1月—2014年12月在青岛大学附属医院口腔颌面外科收治的T2DM-OSCC患者(87例)和普通OSCC患者(113例),利用免疫组织化学(IHC)方法检测2组肿瘤及癌旁组织中chemerin的表达,采用SPSS 26.0软件包对数据进行统计学分析。结果: T2DM-OSCC患者肿瘤组织中chemerin的表达显著高于癌旁组织(P<0.05),其高表达与颈淋巴结转移、TNM分期、肿瘤分化程度和肿瘤复发相关(P<0.05)。T2DM-OSCC患者肿瘤组织中chemerin表达显著高于普通OSCC患者(P<0.05)。合并2型糖尿病chemerin高表达组术后5年总生存率显著低于其他组(P<0.05),肿瘤组织中chemerin表达是影响T2DM-OSCC患者5年总生存率的独立预后因素。结论: Chemerin在T2DM-OSCC中的表达较普通OSCC高,其高表达提示T2DM-OSCC患者的预后更差。Chemerin有望成为T2DM-OSCC患者判断预后的潜在指标和治疗靶点。

Abstract:PURPOSE: To explore the expression and clinical significance of chemerin in oral squamous cell carcinoma (OSCC) patients with type 2 diabetes (T2DM), analyze the relationship with clinicopathological characteristics and prognosis, in order to provide evidence for further postoperative treatment of OSCC patients with T2DM (T2DM-OSCC). METHODS: OSCC specimens combined with type 2 diabetes and without type 2 diabetes (general OSCC) were selected from the Department of Oral and Maxillofacial Surgery of the Affiliated Hospital of Qingdao University from January 2009 to December 2014. Chemerin expression levels in tumors and para-cancerous tissues were detected by immunohistochemistry (IHC), SPSS 26.0 software package was used for data analysis. RESULTS: In the T2DM-OSCC patients, chemerin expression was significantly higher in tumor tissues compared with para-cancerous tissues (P<0.05) and chemerin expression was associated with cervical lymph node metastasis, higher TNM stage, lower tumor differentiation, and tumor recurrence (P<0.05). Chemerin expression in tumor tissues of T2DM-OSCC patients was significantly higher than that in ordinary OSCC patients (P<0.05). Log-Rank analysis and Kaplan-Meier analysis showed that the 5-year OS was shorter in the group of high chemerin expression combined with type 2 diabetes than other three groups(P<0.05). Cox model showed that chemerin expression in tumor tissues was an independent prognostic factor (P<0.05) affecting OS of T2DM-OSCC patients. CONCLUSIONS: Chemerin expression was higher in T2DM-OSCC patients than in ordinary OSCC, high expression of chemerin suggests poor prognosis in T2DM-OSCC patients. Chemerin is expected to be a potential indicator and target molecule for judging the prognosis of T2DM-OSCC patients.
