三维CT血管造影在颅内动脉瘤诊断中的价值

摘    要:目的:探讨16排螺旋CT血管造影在颅内动脉瘤诊断中的应用价值。材料与方法:对临床高度怀疑有颅内动脉瘤或非外伤蛛网膜下腔出血患者54例均行16排螺旋CT血管造影,进行图像后处理:采取容积重建(VRT)、最大密度投影(MIP)及多平面重组法(MPR)。同时与手术或数字减影血管造影(DSA)结果:作对比。结果:CTA的图像后处理方法:中,VRT方法:对动脉瘤的检出率最高,<10mm的容积重建术相比DSA动脉瘤的检出率,经检验比较,t=1.998,P>0.05,差异不显著,无统计学意义。3D-CTA对动脉瘤的检出方面相比DSA,经检验比较,t=1.998,P>0.05,差异不显著,无统计学意义。结论:3D-CTA为一种微创伤的颅内动脉瘤诊断方法:,可较好对颅内动脉瘤特征及其与周围组织解剖结构的三维关系清晰显示,在颅内动脉瘤诊断与治疗中有显著优越性。

Abstract:Purpose:To investigate the 16-row spiral CT angiography in the diagnosis of intracranial aneurysms. Materials and Methods:The clinical high index of suspicion of intracranial aneurysms and non-traumatic subarachnoid hemorrhage, 54 patients underwent 16-slice spiral CT angiography and image post-processing to take volume rendering(VR), maximum intensity projection(MIP) and multi-planar reconstmction method(MPR),at the same time with the surgery or digital subtraction angiography(DSA) for comparison.Results-.The method of CTA images,the VR method the highest detection rate of aneurysms < 10 mm volume reconstmction compared to DSA aneurysm detection rate,after examination,the difference was not significant,no significant,t = 1.998,P > 0.05.3D-the CTA for the detection of aneurysms compared to the DSA,the difference is not significant,no significant(t = 1.998,P> 0.05).Conclusion:3D-the CTA is a minimally invasive diagnosis of intracranial aneurysms method may be better to clearly show intracranial aneurysm characteristics of threedimensional relationship with the surrounding tissue anatomy,diagnosis and treatment of intracranial aneurysmssignificant superiority.
