腔隙性脑梗塞MRI与临床的对照分析

摘    要:目的:通过腔隙性脑梗塞的MR表现与临床体征的对照研究,进一步提高对腔隙性脑梗塞的认识。材料与方法:采用MR常规轴位自旋回波(SE)序列,T1,T2加权像及快速液体衰减翻转恢复(FLAIR)序列扫描的病例,回顾性分析198例腔隙性脑梗塞病人的MR及临床表现,把有明确卒中病史的128例归为第一组;未发现有神经学体征的70例归为第二组,进行对比分析。结果:腔隙性脑梗塞分布在大脑深部的位置,包括放射冠、基底节、内囊、丘脑区及脑干等。较多病例为多发性病灶,病灶发生在脑内重要的功能位置,可表现出典型的临床症状。T1WI呈等或稍低信号,T2WI和FLAIR呈高信号。结论:MRI是诊断腔隙性脑梗塞的最有效方法,FLAIR成像能获得比SE T2WI更重的T2加权图像,还抑制了脑脊液信号,提高对脑室周围病灶的检出率。同时认为,弥漫性脑血流减少是产生腔隙性脑梗塞的原因之一。

Abstract:Purpose:The comparison research of cerebral lacunae infarction with MRI findings and clinical symptom,further raise the understanding that cerebral lacunae infarction.Materials and Methods:Adopt conventional spin-echo(SE) MRI and fluid-attenuated inversion recovery(FLAIR) MRI.The MRI findings and clinical data in 198 cases with cerebral lacunae infarction were analyzed retrospectively.There are divided two group for contrast analysis,the group l 128 cases have definitude symptom of stroke;the group 2 70 cases did not discover to have the neurology symptom.Results:The position of the cerebral lacunae infarction distribute in the brain of the deep department,including project the hat,basal ganglia,internal capsule,thalami and brain stem.There are multi-focus in more patients.Important function position take place in the brain of focus,can express a clinical symptom of typical model.The T_1WI prestents equipotent or slightly low signals,the T_2WI and FLAIR presents the high signal.Conclusion:MRI is an most effectively method for the diagnosis of cerebral lacunae infarction.FlAIR can get heavier T_2WI than SE T_2WI and suppressing the signals of the cerebrospinal fluid,raise to check a rate from the periventricular region focus.At same time,widespread decrease of brain blood is one of the reason in produces cerebral lacunae infarction.
