B超引导逆行锁骨下臂丛神经阻滞在急诊手术中的应用效果

目的探讨B超引导逆行锁骨下臂丛神经阻滞在急诊手术中的应用效果。方法将2018年1月至2018年12月于我院行急诊上肢手术的80例患者随机分为观察组和对照组,每组40例。观察组接受B超引导下行逆行锁骨下臂丛神经阻滞,对照组接受肌间沟臂丛神经阻滞。比较两组的麻醉效果。结果观察组的正中神经、尺神经、臂内侧皮神经、前臂内侧皮神经支配区域麻醉起效时间均短于对照组,差异具有统计学意义(P<0.05)。观察组的正中神经、尺神经、臂内侧皮神经、前臂内侧皮神经完全阻滞率均高于对照组,差异具有统计学意义(P<0.05)。结论B超引导逆行锁骨下臂丛神经阻滞的效果较肌间沟阻滞好,其起效时间快、阻滞完善,是适用于急诊手术的上肢麻醉方式。

Application effect of B-ultrasound-guided retrograde infraclavicalar brachial plexus block in emergency operation

Objective To investigate the application effect of B-ultrasound-guided retrograde infraclavicalar brachial plexus block in emergency operation.Methods From January 2018 to December 2018,80 patients who underwent emergency upper extremity surgery in our hospital were randomly divided into observation group and control group,with 40 cases in each group.The observation group received B-ultrasound-guided retrograde infraclavicalar brachial plexus block,and the control group received intermuscular groove brachial plexus block.The anesthesia effect of the two groups was compared.Results The onset time of anesthesia in the controlled area of median nerve,ulnar nerve,medial brachial cutaneous nerve,medial forearm cutaneous nerve of the observation group were shorter than those of the control group,the differences were statistically significant(P<0.05).The complete block rate of median nerve,ulnar nerve,medial brachial cutaneous nerve,medial forearm cutaneous nerve in the observation group were higher than those in the control group,the differences were statistically significant(P<0.05).Conclusion B-ultrasound-guided retrograde infraclavicalar brachial plexus block has a better effect than intermuscular groove brachial plexus block,it has a fast onset time and a perfect block,which is an anesthesia method suitable for emergency surgery.
