产前超声检查与磁共振成像在胎儿脑室孔闭塞综合征诊断中的对比研究

摘    要:目的探讨产前超声检查与磁共振成像(MRI)在胎儿第4脑室孔闭塞(Dandy-Walker)综合征诊断效果的对比。方法选择2011年3月至2015年2月该院住院分娩,且采用超声疑诊胎儿后颅窝结构异常病例48例,均进行产前超声与MRI检查。结果超声声像图表明小脑蚓部缺失,小脑半球分开,第4脑室和后颅窝池相同并扩张;小脑蚓部超声各测量值随孕周增加而增加,且与孕周呈正相关,相关系数为0.975(宽度)、0.966(前后径)、0.984(上下径)(P0.05)。MRI声像图显示小脑延髓池与第4脑室彼此相通及空间相对增大,小脑半球分离,后颅窝明显扩大,幕上脑积水。以尸检或随访获得的最终诊断结果为标准,超声和MRI技术对Dandy-Walker综合征的检出率比较,差异无统计学意义(P0.05)。结论产前超声检查与MRI对胎儿Dandy-Walker综合征诊断能有效反映胎儿生长发育状况,均有较好的声像图特征,具有较高的诊断价值。

The comparative analysis of the diagnostic prenatal ultrasound and MRI in fetal Dandy-Walker syndrome
Abstract:Objective To investigate the comparative effects of the diagnostic prenatal ultrasound and MRI in fetal Dandy‐Walker syndrome .Methods 48 cases of suspected fetal structural abnormalities of the posterior fossa were given the prenatal ultrasound and MRI diagnosis that were selected from March 2011 February 2015 in our hos‐pital .Results Ultrasound images showed that cerebellar vermis were absent and cerebellar hemispheres was separa‐ted ,besides ,fourth ventricle and posterior fossa were identical and expanded ;The cerebellar vermis ultrasound meas‐ured values were positively correlated with acorrelation coefficient of 0 .975 (width) ,0 .966 (anteroposterior diame‐ter) and 0 .984 (P< 0 .05)(vertical diameter) that were increased with the number of pregnant age .MRI sonogram showed that the cistern and the fourth ventricle were connected with each other and the space was relatively large . Morever ,cerebellar hemispheres was separated and posterior fossa was significantly enlarged with ,hydrocephalus . The standard of the final diagnosis was obtained by autopsy or followed‐up .the detection rate of contrast ultrasound and MRI technology differences Dandy‐Walker syndrome compared had no statistical significance (P> 0 .05) .Conclu‐sion Prenatal ultrasound and MRI in the diagnosis of fetal Dandy‐Walker syndrome can effectively reflect fetal growth and development and both of them had better image features and better diagnostic value .
