腹部手术后胃瘫的诊断与治疗

目的 总结腹部手术后胃瘫的临床诊治经验。方法 对腹部手术后并发胃瘫的63例患者临床资料进行回顾性分析。结果 术后胃瘫的临床表现主要为腹胀、呕吐，无明显腹痛，大多数患者排气正常。消化道X线钡餐造影示胃蠕动差，胃镜检查吻合口通畅。63例经胃肠减压、营养支持、维持水电解质代谢平衡以及应用促进胃肠蠕动的药物及中西医结合等非手术治疗均缓解。结论 手术后胃瘫的诊断主要依据临床表现；胃镜检查有其重要价值；排除机械性梗阻后，采取非手术治疗，可取得满意疗效。

Diagnosis and Treatment of Postoperative Gastroparesis following Abdominal operation

Objective To summarize the clinical experiences in diagnosis and treatment of gastroparesis after abdominal operation. Methods The clinical data of 63 eases with grastroparesis after abdominal operation were analyzed retrospectively. Results The clinical manifestation of postoperative grastroparesis was abdominal distention,vomit, no obvious eeliodynia and normal gas discharge by anus in majority patients, weak enterocinesia was shown by barium meal X-ray of alimentary canal opaeifieation, and unobstructed anastomotie stoma shown by gastroscopy. 63 patients relieved from gastrointestinal decompression, nutritional support, water-electrolyte metabolism balance,gastrointestinal motility medicine and combined integrated traditional chinese medicine and western medicine treatment. Conclusion Postoperative grastroparesis mainly diagnosis depends on clinical manifestation. Gastroseopy is of significant value in diagnosis of grastroparesis. When mechanical intestinal obstruction is excluded, non-surgieal treatment way should be adopted and would have satisfactory therapeutic effect.
