新生儿危重病与胃肠功能障碍关系的探讨

目的探讨新生儿危重病与胃肠功能障碍的关系。方法评估 73例危重新生儿危重程度、多器官功能衰竭与胃肠功能障碍的关系。结果 73例危重新生儿中单项危重病 12例 ,多项危重病 6 1例 (其中危重 33例、极危重 2 8例 ) ;6 1例中 12例为单器官功能衰竭 ,余 4 9例为伴多器官功能衰竭。胃肠功能障碍发生率在危重病组与极危重病组分别为39.4 % (13/ 33)、71.4 % (2 0 / 2 8) ,两组比较具有显著性差异 (P <0 .0 5 )。单项危重病组与伴多器官功能衰竭组胃肠功能障碍发生率分别为 16 .7% (2 / 12 )、5 9.2 % (2 9/ 4 9) ,两组比较有非常显著性差异 (P <0 .0 1)。胃肠功能障碍的危重新生儿平均伴有器官衰竭的数目为 1.83个。结论新生儿病情越重 ,受损器官越多 ,胃肠功能障碍发生率越高 ,因此 ,要高度警惕 ,早期发现 ,早期治疗。

The relationship between critical ill neonates and gastrointestinal dysfunction

Objective To study the relationship between critical ill neonates and gastrointestinal dysfunction. Methods The relationship between the severity of illness or multiple organ failure(MOF) and gastrointestinal dysfunction was assessed in 73 critical ill neonates. Results Of 73 critical neonates, 12 suffered from critical illness without MOF and 61 suffered from multi-critical illness(33 serious, 28 extremely serious). Among 61 patients, 12 were accompanied with single organ failure and 49 with MOF. The morbidity rate of gastrointestinal dysfunction was 39.4%(13/33) and 71.4%(20/28) in the serious illness and extremely serious group, respectively, with significant difference(P<0.05), which was 16.7%(2/12) and 59.2%(29/49),respectively, in the group without MOF and in the MOF group, with very significant difference(P<0.01). The number of gastrointestinal dysfunction accompanied by organ failure was 1.83 on average. Results The more serious the neonatal illness is, the more number of organ failure accompanied and the higher morbidity rate of gastrointestinal dysfunction. Therefore, more attention should be paid to the gastrointestinal dysfunction so that gastrointestinal dysfunction will be discovered and treated early.
