经桡动脉行冠心病介入治疗在经济欠发达地区应用前景的研究

目的 探讨经桡动脉行冠心病介入检查及治疗在经济欠发达地区的应用前景.方法 连续统计2005年8月至2007年6月于我院行经桡动脉途径冠状动脉造影及冠状动脉介入治疗的病例资料220例,观察手术成功率、并发症及住院总费用,与文献荟萃资料进行对比分析.结果 220例患者均首先采取经桡动脉穿刺行冠状动脉造影检查及介入治疗,桡动脉穿刺成功217例,成功率98.6%;失败3例经股动脉穿刺,介入检查及治疗总成功率100%.桡动脉穿刺成功217例患者中冠状动脉造影阴性者74例,另143例显示有明确冠状动脉病变(管腔狭窄≥70%),其中单只病变50例(35.1%),双只病变71例(50.4%),多只病变22例(14.5%),对229处靶病变进行介入治疗,植入支架265枚,无严重并发症发生,术后住院(2.7±1.8)d,经桡动脉行冠心病介入检查及治疗患者的住院总费用为人民币5065～82 677元,平均(30 566±25 208)元,与文献荟萃资料进行对比均无统计学差异.结论 经桡动脉行冠心病介入检查及治疗是一种安全、有效、治疗费用低、患者容易接受的治疗方法,在经济欠发达地区有广泛的应用前景.

The application future of transradial coronary intervention in under-development region

Objective To discuss the application future of transradial coronary intervention in under-development region.Methods 220 patients underwent the transradial coronary arteriongraphy and intervention treatment from 2005 to 2007.The success rate,complications and hospital cost were compared with literature.Results At the beginning 220 cases underwent transradial coronary arteriongraphy and intervention treatment,including success in 217 cases with success rate of 98.6% and failed transartery pierce in 3 cases.The success rate of intervention examination and treatment reached 100%.In the 217 successful cases,74 cases presented negative coronary arteriongraphy,143 cases had clear pathological changes,including 50 single changes,71 double changes and 22 muhichanges.Through intervention treatment to targeted changes in 229 cases,transplanting 265 intents,there was no complication.The patients would be in hospital for(2.7+1.8 days)after operations,and the total medical fees of the transradial coronary artefiongraphy and intervention treatment were RMB 505682677,which was no significant difference with the documentary materials.Conclusion The transradial coronary arteriongraphy and intervention treatment is a safe,effective,economical and acceptable medical method,which has wide application future in underdevelopment regions.
