全髋/全膝关节置换术后患者应用足底动静脉脉冲系统减轻患肢肿胀的效果观察

摘    要:目的探讨应用足底动静脉脉冲系统(足底A—V泵)减轻全髋/全膝关节置换术(THA/TKR)后患者患肢肿胀的效果。方法全髋人工关节置换术患者22例(A组),全膝人工关节置换术后患者36例(B组)。经过患者知情同意,将A组患者随机分为治疗组A12例,对照组A10例,B组患者分为治疗组B18例,对照组B18例。治疗组术后立即应用足底A—V泵,持续24h应用,连续7d;对照组除了不使用足底A—V泵以外,其他治疗、护理和功能锻炼方法与治疗组一样。术前1d晨、术后1-3d晨、术后7d晨分别测量下肢周径,了解其肿胀程度。结果两种术式下,治疗组在下肢胀肿减轻程度方面效果均优于对照组(P〈0.05);患者均无出血并发症;对照组B中有2例患者发生近端深静脉血栓形成。结论应用足底A—V泵加功能锻炼在减轻全髋/全膝人工关节置换术后下肢肿胀方面具有明显的效果,并可减少下肢深静脉血栓形成,而且此项操作简单方便,安全高效。

Abstract:Objective To explore the efficacy of plantar arteriovenous pulse system(A-V pulsing) in reducing limb swelling induced by total hip/knee rcplacement(THA/TKR). Methods Group A consisted of 22 patients undergoing THA, while Group B, 36 patients with TKR. By their consent, Group A were subdivided into the treatment group (n=12) and the control group (n=10), and Group B, into the treatment group (n=18) and the control group (n=18). Immediately after the operation, the treatment group were offered plantar A-V pulsing 24 hours a day for 7 running days, while the control group were not, with the other treatments, nursing and function exercising being the same as the treatment group. On the day before the operation, the first to third days, and the 7th day after the operation, measurement of the circumferences the lower extremities was done for the understanding of the swelling. Results The swelling among the treatment group was distinctly less serious than that among the control group (P〈0.05), with no complications of hemorrhage occurring among the treatment group, but among the control group, there occurred two cases of proximal deep vein thrombosis. Conclusion Application of plantar A-V pulsing complemented by function excercising is effective in alleviating swelling in the lower extremes and preventing deep vein thrombosis, and at the same time, easy to operate and safe.
