社区2型糖尿病患者自我管理水平调查与健康教育对策

目的调查社区2型糖尿病患者糖尿病自我管理水平,以期为进一步开展糖尿病专科护士社区糖尿病健康教育工作提供科学依据。方法 2009-2011年,便利抽样法选取合肥大兴镇、西园、花岗、胜利等4个社区卫生服务中心确诊的2型糖尿病患者344例为研究对象,采用自制一般情况调查表及自我管理水平调查问卷对其进行调查。结果本组患者使用胰岛素者占55.52%,其中不会正确注射胰岛素者43.46%;饮食结构不合理、不能定期检测血糖,不能定期随访、足部保健知识缺乏及不能坚持运动者均占到50.00%以上;空腹血糖达标率为28.20%,早餐后2h血糖达标率为22.97%。结论糖尿病专科护士应针对社区2型糖尿病患者自我管理存在的问题,实施针对性健康教育,以提高患者自我管理水平。

Investigation of Self-Management Level and Health Education Strategies of Type 2 Diabetes Patients in Community

Objective To investigate the self-management level of patients in community with type 2 dia- betes,and in order to provide the scientific basis for further development of community diabetes health ed- ucation for diabetes specialist nurses.Methods By convenience sampling, 344 patients with type 2 diabetes were selected and surveyed by the self-made general questionnaire and self management level survey ques tionnaire.Results 55.52% of patients were treated by insulin,but 43.46% of patients can not insulin injec- tions correctly.The patients who had unreasonable dietary pattern, not monitor blood glucose regularly, irregularly follow-up, lack of foot health knowledge and can not adhere to exercise accounted for more than 50%. Fasting blood glucose success rate was 28.20% and fasting glucose after breakfast 2 h blood glucose success rate was 22.97%.Conclusion Diabetes specialist nurses should implement the targeted health edu- cation to type 2 diabetes patients aiming at improving patients self management level.
