产科护士门诊病房一体化模式对妊娠期糖尿病患者妊娠结局、焦虑及满意度的影响

目的探讨产科护士门诊病房一体化模式对妊娠期糖尿病患者妊娠结局、焦虑及满意度的影响,以期为产科护理学科的发展与完善提供一定依据。方法 2012年5月1日至2013年4月30日,便利抽样法选择上海市某三级甲等综合性医院产科建卡并定期产检的妊娠期糖尿病孕妇304例为研究对象。按随机数字表法将其分为观察组(n=150)和对照组(n=154)。观察组患者除接受常规产前门诊外,还参加由产科病房资深护士开设的门诊,并继续接受产后服务即对应责任制护理;对照组患者只进行常规产前门诊。比较两组患者有关妊娠结局、焦虑及满意度等指标。结果观察组患者入组时和分娩前焦虑程度的差异无统计学意义(P0.05),对照组患者分娩前的焦虑程度高于入组时,差异有统计学意义(P0.05)。观察组患者自然分娩率和患者满意率均高于对照组,差异均有统计学意义(均P0.05)。观察组产妇新生儿体质量低于对照组,差异有统计学意义(P0.05)。结论产科护士门诊病房一体化模式可以降低妊娠期糖尿病产妇的焦虑水平,优化分娩结局,提高自然分娩率和孕妇围产期满意度。

Effects of the Integration Model of Obstetric Nurse,Prenatal Outpatient,and Ward on the Preg-nancy Outcome,Anxiety of Women and Their Satisfaction Degree

Objective To explore the effects of a integration model of obstetric nurse,prenatal outpatient, and ward on the pregnancy outcome,anxiety of women with gestational diabetes mellitus and their satis-faction degree,in order to provide a basis for the development and perfection of obstetrical nursing.Meth-ods By convenience sampling,304 pregnant women with gestational diabetes mellitus were selected and randomly divided into observation group (n =150)and control group (n =154).The observation group re-ceived prenatal checkups conducted by senior nurse from obstetric ward on the basis of routine prenatal checkups,the ward nurse also continued to follow up the post-natal services corresponding to the nursing responsibility system.The control group received only routine prenatal checkups.The pregnancy outcome, anxiety,and satisfaction degree were compared between two groups.Results There was no statistical sig-nificance of anxiety degrees between the initial time of the observation group and the time before delivery (P >0.05).The anxiety degree before delivery was higher than the initial time in control group(P <0.05). The natural births rate and satisfaction degree in the observation group were higher than that of control group (all P <0.05).The birth weight of newborns in the observation group was lower than that of the control group (P < 0.05 ).Conclusion This integration model of obstetric nurse,prenatal outpatient and ward could decrease the anxiety degree of puerperal,optimize the pregnancy outcome,and improve the rate of natural birth and satisfaction degree of perinatal period.
