以躯干为主导的矫正脑卒中下肢非典型异常运动模式的疗效分析

摘    要:目的:以神经生理学的交互抑制理论为指导,探讨纠正脑卒中下肢呈非典型运动模式的方法。方法:从当地医院住院1个月的348例脑卒中患者中,共选出下肢呈非典型伸肌运动优势模式和屈肌运动优势模式43例(约12.36%)。随机分成实验组24例和对照组19例。两组均采用常规的ROM训练、ADL训练等方法进行,但实验组增加了交互抑制理论指导的训练,然后比较康复治疗前后的效果。结果:两组的Fugl-Meyer下肢功能积分、Carr-Shepherd平衡评分和Barthel指数均提高(P<0.01),并且实验组明显优于对照组(P<0.01)。结论:以躯干为主导的矫正异常运动模式的方法,能明显改善脑卒中下肢屈曲模式及非典型伸肌模式,有利于提高肢体随意运动、平衡能力及步行模式。

Abstract:Objective:To find a new method of remedying the atypical motor pattern in stroke patient's lower limb directed by neural reciprocal inhibiting theory.Method:43 patients with lower limb atypical extensor motor dominant pattern and flexor motor dorminant pattern(about 12.36%) were recruited from 348 stroke inpatients for 1 month in the local hospital, and were randomly divided into two groups,experiment group (24 cases) and control group(19 cases).Both of them were received regular ROM and ADL training,while the experiment group were accepted additional treatment directed by reciprocal inhibiting theory,then compared the effect of treatment before and after therapy.Result:The score of Fugl-Meyer's lower limb assessment,Carr-Shephard's balance function assessment and Barthel's index in the two groups were improved (P<0.01), and the result of the experimental groups were better than that of the control group(P<0.01). Conclusion:The method of remedying limb a typical motor pattern directed by trunk dominat theory can remarkably amend the lower limb flexor pattern and a typical extensor motor pattern in stroke patients, and obviously improve the ability of the voluntary movement,balance ability and walking pattern.
