原发性高血压患者血压与胰岛素抵抗的关系研究

目的探讨原发性高血压患者血压与胰岛素抵抗的关系。方法1680例健康查体者在排除了高血压史、冠心病、脑血管病、糖尿病等疾病及既往史后选取68例，根据其血压状态分为正常血压组（A组，收缩压〈120mmHg，舒张压〈80mmHg）34例和正常高值组（B组，120mmHg≤收缩压〈140mmHg或80mmHg≤舒张压〈90mmHg）34例2组；另选择住院原发性高血压患者73例为高血压组（c组，收缩压≥140mmHg或舒张压≥90mmHg）。C组依血压程度分为C1、C2、C33个亚组。测定各组空腹血糖（FBG）、空腹血清胰岛素（FINS）。结果胰岛素抵抗指数，B组与A组比较差异无显著性意义（P〉0．05）；C组与A组和B组相比则差异均具有显著性（均P〈0.01）；C1、C2及C3组之间相比差异均有显著性（均P〈0.01）。结论原发性高血压患者存在明显的胰岛素抵抗，且血压越高胰岛素抵抗越明显。

The Relationship between Blood Pressure and Insulin Resistance in Primary Hypertension Patients

Objective To investigate the relationship between insulin resistance and blood pres sure in primary hypertensive patients. Methods Sixty-eight patients without hypertension history, coronary artery disease, brain angiopathy, prostatomegaly,etc were selected from 1 680 persons who had taken healthy examination. The subjects were divided into two groups according to their value of blood pressure. There were 34 people in the normal control group （group A,systolic pressure〈120 mm Hg and diastolic pressure〈80 mm Hg）and 34 people in the prehypertension group （group B, 120 mm Hg≤systolic pressure〈140 mm Hg or 80 mm Hg≤diastolic pressured 90 ram Hg）. We detected 73 patients with primary hypertension in our hospital and selected them into the hypertension group （group C,systolic pressure≥140 mm Hg or diastolic pressure≥90 mm Hg）. The group C were divided into C1,C2 and C3 groups according to their value of blood pressure. Their fasting blood glucose （FBG） and fasting insulin （FINS） were determined. Results Compared with the group A,the insulin resistance index （IRI） did not show statistics difference in the group B（P〉0.05）. Compared with the group A and B, however, it was remarkably advanced in the group C（P〈0.01）. The IRI did show statistics difference in the groups of C1 ,C2 and C3 （P〈0.01）. Conclusion There are remarkable insulin resistance in primary hypertension patients. The higher the blood pressure is,the more obvious the IRI is.
