地塞米松治疗重症急性胰腺炎大鼠胰性脑病的研究

摘    要:【目的】探讨地塞米松治疗重症急性胰腺炎(SAP)大鼠胰性脑病的治疗效果。【方法】将实验用大鼠随机分成3组,采用改良AhoS法制作SAP模型。A组为地塞米松治疗组:SAP制模后3h给予大鼠地塞米松治疗;B组为SAP大鼠空白对照组、C组为假手术组。分别检测各组模型制作成功后24h的血清髓鞘碱性蛋白(MBP),血清淀粉酶(AMS)、白细胞介素-6(IL-6)水平及脑水含量和脑组织细胞超微结构的变化。【结果】B组中大鼠血清AMS、MBP、IL-6、脑水含量水平显著高于C组与A组,且差异均有显著性(P均〈0.05)。C组大鼠脑细胞超微结构正常,无病变;B组大鼠神经元细胞核出现明显的边集现象,线粒体、内质网均出现不同程度的变性;A组大鼠脑细胞线粒体有轻度肿胀,细胞核、内质网无异常。【结论】地塞米松可有效的控制SAP炎症的发生扩散,降低胰性脑病发生率。

Study on Dexamethasone for the Treatment of Pancreatic Encephalopathy in Rats with Severe Acute Pancreatitis

Abstract:Objective]To explore the efficacy of dexamethasone for the treatment of pancreatic encephalopathy in rats with severe acute pancreatitis(SAP). Methods] Experimental rats were randomly divided into 3 groups. SAP model was made by using Ahos method. Rats in group A( dexamethasone group) were given dexamethasone 3h after SAP model establishment. Group B was SAP blank control group. Group C was sham operation group. Serum myelin basic protein(MBP), amylase(AMS), interleuki-6 (IL6), brain water content and brain tissue of each group 24h after model establishment were measured. Results] Serum levels of AMS, MBP, IL-6 and brain water contents in grotip B were significantly higher than those in group C and group A, and there were significant difference(all P d0.05). The microstructure of brain cells in rats of group C was normal and had no lesion. Obvious side aggregation in neuron cells and the denaturation of mitochondria and endoplasmic reticulum were observed in rats of group 13. Mitochondria of brain cell in rats of group A had mild swelling and no abnormal nucleus and endoplasmic reticulum. Conclusion] Dexamethasone can effec tively control the occurrence and expansion of SAP inflammation, and reduce the incidence of pancreatic encephalopathy.
