萘呋胺酯治疗弥漫性脑损伤的实验研究

摘    要:【目的】观察萘呋胺酯(NF)对弥漫性脑损伤(DBI)的治疗作用。【方法】采用Marmarou的DBI大鼠模型,光镜下测量顶叶皮质内微血管及其周围水肿区的截面面积.电镜下半定量测定皮质区毛细血管周的水肿范围及胶质细胞胞体的肿胀程度.腹腔注射NF50mg/kg,1次/8h,观察其对上述指标的影响。【结果】外伤后2h、6h、24h平均微血管截面面积与对照组比较均显著缩小,6h、24h微血管周水肿明显增加。NF治疗组伤后2h微血管无扩张,但其周围水肿增加,24h两者均改善。电镜下2h、24h毛细血管周水肿明显;胶质细胞胞体肿胀在2h明显.24h无显著差异;NF在2h能减轻胶质细胞肿胀程度.但对毛细血管周水肿范围影响不大。【结论】外伤后2hNF能加重血管源性脑水肿,但能改善细胞毒性脑水肿。而在24h能改善血管源性脑水肿。故在外伤早期应慎用血管扩张药物。

Abstract:Objective]To observe the treatment effects of naftidrofuryl(NF) on diffuse brain injury(DBI) in rats by morphometric method.Methods]Using Marmarous DBI rat model,the microvascular cross section area and perivascular edema area in parietal cortex were calculated under light microscope,the edema range of pericapillary and swelling extent of astrocytes were observed and semi-quantitively measured under transmission electron microscope.Treated with NF 50 mg/kg,q 8 hr injected to abdominal cavity and observed its effects on the above indexes. Results]Compared with control group,the average microvasculature cross section of DBI at 2,6,24 hr after injury was reduced obviously and the average perivascular edema area at 6,24 hr after injury was increased markedly.Treated with NF,there was no enlargement in the microvascular cross section at 2 hr after insult but the vasogenic brain edema was worse,and at 24 hr after insult.the dilatation of vessels and the brain edema were alleviated.Under EEM,the pericapillary edema could be observed at 2 hr and 24 hr after insult.Astrocytes were swelling at 2 hr but recovered at 24 hr,NF could alleviate astrocytic swelling but played no role in reducing edematous range of pericapillaries.Conclusion]NF could aggravate the vasogenic brain edema but alleviate cytotoxic brain edema at 2 hr after insult and could alleviate cytogenous brain edema at 24 hr.The use of vasodilator should be cautious at early stage of head injury.
