摘    要:目的探讨联合氧疗治疗有合并症的重型颅脑损伤患者的效果与监护措施。方法:197例有合并症的重型颅脑损伤患者随机分为联合氧疗组(A组)137例和高压氧(HBO)治疗组(B组)60例,分别采用早期常压下面罩或吸氧头罩吸纯氧 HBO联合疗法和HBO疗法两种方案治疗,比较两组疗效,观察患者对治疗的反应适应性。结果:A组治疗效果、治愈率明显优于B组(P<0.01),开始治疗的时间也明显早于B组(P<0.05),HBO治疗时A组心率、血压、呼吸的变化与B组比较差异有显著性(P<0.01),治疗后两组血氧饱和度均明显提高,心律失常好转,差异无显著性(P>0.05)。结论:对有合并症的重型颅脑损伤患者早期采用联合氧疗治疗,可明显提高患者治愈率和生存质量。

Abstract:Objective :To explore the effectiveness and monitoring measures of combined oxygen therapy in the treatment of severe craniocerebral injury patients accompanied by complications.Methods:197 cases were divided into combined oxygen therapy group(group A)and hyperbaric oxygenation group(group B),group A was treated with an early mask or head protector oxygen inhalation,and added hyperbaric oxygenation,group B was treated with hyperbaric oxygenation.The efficacy was compared between the two groups,the reaction and adaptability of patients were investigated.Results:Both the efficacy rate and the healing rate were higher in group A compared to group B(P<0.01),and the treatment of group A began was earlier than group B(P<0.05).In the treatment of HBO,changes of heart rate,blood pressure and respiratory rate were significant difference between the two groups(P<0.01).Oxygen saturation rate was increased and the arrhythmia improved with no significant difference between the two groups(P>0.05).Conclusion:Combined oxygen therapy at early period was an effective measure to accelerate the recovery of brain function and raising the survive quality in severe craniocerebral injury patients.
