定量组织速度成像对心肌梗死患者心肌速度梯度的研究

摘    要:目的 应用定量组织速度成像(QTVI)分析正常人及心肌梗死患者的心肌运动速度及心肌速度梯度(MVG),探讨MVG评价缺血心肌局部功能的价值。方法 离线分析19例心肌梗死患者和13例正常对照者左室后壁及前室间隔的心肌多普勒速度曲线,分别测量收缩期及舒张早期短轴方向和长轴方向上的心肌运动速度,并计算相应的MVG。结果 心肌梗死时缺血节段在短轴方向上仅前室间隔的心肌速度较正常明显下降,长轴方向上心肌运动速度较正常无明显变化,而短轴方向和长轴方向上MVG均较正常显著降低。结论 MVG是比心肌运动速度更敏感的反映局部心肌缺血的指标。

Abstract:Objective To quantitatively analyze myocardial velocity and myocardial velocity gradient(MVG) in patients with myocardial infarction (MI) by quantitative tissue velocity imaging and evaluate the value of MVG in assessing regional function of ischemic myocardium.Methods There were 19 patients with MI and 13 healthy controls in this study. The myocardial velocities of left ventricular posterior wall and anteroseptum in systole and early diastole on both longitudinal and radial axis were measured with QTVI. MVG were calculated respectively. Results Comparing with the healthy controls, velocity of the ischemic myocardium in the patients with MI was significantly decreased on radial axis, but only at region of anteroseptum, while the velocity was no significant change on the longitudinal. MVG on both the radial and the longitudinal was remarkably decreased.Conclusion MVG is more sensitive than myocardial velocity for detecting ischemic myocardium.
