超声与MRCP在诊断胰胆系疾患中价值的ROC分析

摘    要:目的 用Receiver Operating Characteristic curves(ROC)分析评价超声和磁共振胰胆管造影(MRCP)对胰胆系疾患的诊断价值,并简要介绍ROC的原理和应用。方法 怀疑胰胆系疾患行MRCP检查的病人中同时具有完整的超声资料和最终诊断的患者共31例,其中男16人,女15人,范围26~78岁,平均年龄57.0岁作为研究对象。用SPSS11.0软件进行ROC分析。结果 ROC分析显示,超声诊断胆系疾患的ROC曲线下面积(AUS)=0.889,MRCP诊断胆系疾患的ROC曲线下面积(AMRMRCP)=0.924表明MRCP诊断胆系疾患的价值优于超声。结论 对于怀疑胆系疾患的病人,超声是首选的诊断方法,MRCP在诊断胆管下段疾病方面优于超声,是进一步检查的最佳选择。

Abstract:Objective To evaluate the value of diagnostic ultrasound and MRCP in patients with cholangiopancreatic disease by Receiver Operating Characteristic curves (ROC),and introduce the principle of ROC simply. Methods Ultrasound and MRCP were performed in 31 patients (include 16 male and 15 female; ranging from 26-78 years old; mean age 57.0 years) who were suspected with cholangiopancreatic disease. We analysed the data with ROC function of software SPSS 11.0 . Results Area under the ROC curve of ultrasound was (A US )=0.889. Area under the ROC curve of MRCP was (A MRCP )=0.924. ROC analysis showed that effect of diagnosis cholangiopancreatic disease by MRCP was better than by ultrasound. Conclusion Ultrasound is the first choice to diagnosis cholangiopancreatic disease, and MRCP is beneficial complement.
