长骨骨纤维结构不良的MR诊断

摘    要:目的:探讨长骨骨纤维结构不良的MRI表现。方法:回顾性分析15例长骨骨纤维结构不良的MRI表现,其中9例做了增强。结果:单骨型12例:股骨8例,胫骨2例,腓骨1例,肱骨1例;多骨型3例:2同时侵犯股骨和胫骨,1例同时侵犯肱骨和尺度。MR表现:7例T1WI呈不均匀等低信号,T2WI呈不均匀高信号;6例T1WI呈均匀极低信号,其中T2WI上3例为均匀高信号,3例仍为极低信号;2例T1WI和T2WI均为较均匀等信号。所有病灶边界均清楚,其中3例病变周围可见完整或不完整硬化带,全部患者未见明显软组织肿块及骨膜反应。9例增强后表现为不同程度强化,其中3例为均匀强化,6例为不均匀强化。结论:长骨骨纤维结构不良的MRI表现并无特征性,信号特点取决于组织结构。

Abstract:Objective To discuss the findings of fibrous dysplasia of long bone on MRI. Methods The MRI appearances of 15 patients with confirmed fibrous dysplasia of long bone were analyzed retrospectively. Results There were single bone type (12 cases):femur (8),tibia (2),fibula (1),humerus (1),and multiple bone type (3):femur and tibia(2),humerus and ulna (1).Seven cases were showed heterogeneous lower intensity on T1WI and high intensity on T2WI. In the 6 cases who were showed extremely homogeneous low intensity on T1WI,3 were homogeneous high while the other 3 were still extremely low intensity on T2WI. Isointensity on both T1WI and T2WI were found in 2 cases. The margin of all lesions were clear and complete or incomplete ossify strip were shown on 3 cases. No soft tissue or periosteal reaction could be found in all cases. Vary degree enhancement could be seen in 9 cases after administration of contrast agents,including 3 homogeneous and 6 heterogeneous. Conclusion On MRI,fibrous dysplasia of long bone had no unique sign or character signal,which depended on the pathologic elements of the lesion.
