MRI动态增强扫描对垂体微腺瘤的诊断意义

摘    要:目的 评价MRI动态增强扫描探查垂体微腺瘤的效果。方法 研究38例垂体微腺瘤的平扫、Gd-DTPA动态增强扫描及延迟的常规增强扫描的效果。结果 动态和常规增强扫描比较,有9例显示相同;23例动态比常规增强效果好;6例常规比动态增强效果好。11例只有动态增强能显著;2例只有常规增强显示。结论 动态增强扫描可以提高垂体腺瘤的检出率,和常规增强扫描结合可以提高诊断敏感性。

Abstract:Objective To evaluate the effectiveness of dynamic contrast enhancement MRI in the diagnosis of pituitary microadenomas. Methods 38 patients with pituitary microadenoma were stuided by means of Gd DTPA enhanced dynamic MRI and were also scanned with conventional spin echo sequences before and after dynamic scanning. Results The dynamic sequence was judged to be better than the standard enhanced sequence for depicting microadenomas in 23 patients, and 6 lesions were better demonstrated in the conventional contrast enhancement sequence. Lesions were seen equally well on the standard and dynamci sequences in 9 cases. The lesions were identified only on the dynamic sequence in 11 patients,whereas there was 2 cases in which lesions were detected only on the standard postcontrast sequence. Conclusion Dynamic MRI improved lesion detection over standard sequences after contrast infusion. Both sequences are necessary, and combined study of the sequences can improve the detection sensitivity.
