摘    要: 目的 评价MR三维肝脏容积快速采集(LAVA)扫描序列在诊断壶腹周围癌的应用价值。方法 收集经病理证实的17例壶腹周围癌患者的MR T1WI、T2WI、MRCP和胰头-壶腹部LAVA多期动态增强扫描图像。分析不同病理类型壶腹周围癌的影像学特征,探讨MRI特别是LAVA序列在显示壶腹周围癌中的特点。结果 17例患者中,术后经病理证实胰头癌7例,胰头段胆管癌4例,壶腹癌6例。LAVA增强扫描序列能够清晰显示病灶的形态以及与周围组织器官的关系,同时三种病理类型在不同血管时相表现出不同影像学特征。结论 MR LAVA增强序列可以为不同病理类型壶腹周围癌定位、定性诊断提供更加可靠的证据,对于外科手术及预后均有较大的指导意义。

Abstract:Objective To observe the value of MR liver acquisition with volume acceleration (LAVA) sequence in diagnosis of periampullary carcinoma. Methods The MRI data including T1WI, T2WI, MRCP and LAVA sequence of 17 patients with periampullary carcinoma confirmed by pathology were analyzed retrospectively. MRI manifestations of different pathological types periampullary carcinoma were analyzed, the characters of periampullary carcinoma in LAVA sequence were observed. Results Among 17 patients with periampullary carcinoma, 7 were pancreatic head carcinoma, 4 were cholangiocarcinoma in pancreatic head, and 6 were ampullary carcinoma. The shape of lesions and the relationship between lesions and surrounding tissues were clearly showed in LAVA sequence. Different time phases in various pathological types periampullary carcinoma showed different appearances. Conclusion LAVA sequence can provide reliable evidence for localization and qualitative diagnosis of different pathological types of periampullary carcinoma. It is important to surgical operation and prognosis.
