摘    要:目的探讨T2值定量测定在膝关节骨关节炎(OA)影像诊断中的价值。方法采用PHILIPS Acheiva 3.0超导型磁共振仪,8回波SE序列行垂直于髌软骨后缘横断面扫描。运用T2图(T2mapping)成像测量159例髌软骨中层平均T2值,并进行统计学分析。其中正常组(n=76)、OA组(n=83),按照Kellgren-Lawrence评分OA组又分为轻度OA组(n=41)、中度OA组(n=29)及重度OA组(n=13)。结果正常组、轻度OA组、中度OA组及重度OA组平均T2值分别为(39.31±3.90)ms、(48.58±8.53)ms、(54.92±8.84)ms、(59.03±6.30)ms;协变量年龄对T2值有明显的影响(F=5.745,P=0.018<0.05);4组髌软骨T2值有明显差异(F=26.579,P=0.00<0.05)。结论软骨T2值定量测定可以敏感反映出关节软骨形态轮廓无明显改变之前的软骨内生化成分的改变。

Abstract:Objective To observe the diagnostic value of T2 relaxation time in patella cartilage of osteoarthritis (OA). Methods Quantitative T2 mapping of patella cartilage was evaluated with a 3.0T MR imaging spectrometer in 83 patients of OA and 76 healthy subjects. The study population was categorized with radiography and clinical diagnosis as normal group (n=76), mild OA group (n=41), moderate OA group (n=29) and severity OA group (n=13). T2 relaxation times were measured in middle layer of patella cartilage, then statistical differences were analyzed. Results The average T2 relaxation of normal group, mild OA group, moderate OA group and severity OA group were (39.31±3.90)ms, (48.58±8.53)ms, (54.92±8.84)ms and (59.03±6.30)ms, respectively. Significant differences were found among the four groups (F=26.579, P=0.00<0.05), and the covariance age affectted T2 relaxation obviously (F=5.745, P=0.018<0.05). Conclusion T2 relaxation time can indicate component abnormality of articular cartilage without morphology change of early OA.
