摘    要:目的探讨超声组织追踪成像(TTI)和应变率成像(SRI)评价冠心病患者左室收缩功能的临床价值,比较TTI和SRI两种方法评价左室收缩功能敏感性的差异.方法A组22例健康人群,B组20例左前降支和18例右冠状动脉狭窄的冠心病患者,应用组织速度成像(TVI)技术于心尖四腔切面、心尖两腔切面、心尖左室长轴切面上显示后间隔、下壁、前壁及前间隔的图像,并应用TTI及SRI定量测量并分析左室11个节段收缩期峰值位移值及收缩期最大应变率.结果B组各节段的各项指标较A组明显减低.冠心病组总节段数为208个,TTI、SRI检出异常节段数分别为141、126个,检出比例分别为67.79%、60.58%.结论TTI和SRI技术能准确地定量评价冠心病患者的左室收缩功能,对左室收缩功能评价TTI与SRI结果之间无明显统计学差异.

Abstract:Objective To investigate the clinical significance and compare the sensitivity of tissue tracking imaging (TTI) with strain rate imaging (SRI), in evaluating left ventricular systolic function in patients with coronary heart disease (CHD). Methods Twenty-two healthy volunteers (group A), 20 cases of left anterior descending branch stenosis and 18 cases of right coronary artery stenosis(group B) were examined with TTI and SRI. Images of posterior septum, inferior wall, anterior wall and anterior septum were got at apical four chamber view, two-chamber view and apical longitudinal view, respectively. Peak systolic displacement (D) and peak systolic strain rate (SR) of 11 segments were measured with TTI and SRI. Results D and SR were significantly lower in group B than those of the corresponding segments in group A (P<0.01 or 0.05). The detection number of abnormal segments was 141 and 126, and the detection rate of abnormal segments was 67.79% and 60.58% with TTI and SRI respectively. Conclusion TTI and SRI can quantitatively assess left ventricular systolic function in CHD. There is no difference between TTI and SRI for evaluation of left ventricular systolic function.
