直肠癌局部浸润的高分辨率MRI征象与病理学T分期的对照研究

摘    要:目的 探讨直肠癌局部浸润的MRI征象与病理分期的关系.方法 148例经病理证实的直肠癌患者,术前接受MR检查.对直肠癌局部浸润的高分辨率MRI征象进行分析、归类,做出分期诊断,并与术后病理组织学肿瘤T分期进行对照.结果 148例直肠癌MRI T分期总的诊断准确性为90.54%,T1~T4期MRI诊断准确性分别为98.65%、91.22%、91.89%和99.32%.直肠癌侵犯肠周径程度与病理T分期间呈中等正相关(rs=0.59, P<0.001).直肠癌局部侵犯的MRI征象与病理T分期间呈中等正相关(rs=0.62,P<0.001).结论 高分辨率MR对于直肠癌T分期有较高的诊断准确性,有助于制定合理的治疗方案.MRI征象的正确识别和肿瘤对肠管周径侵犯程度的评价对MRI直肠癌T分期诊断有一定的帮助.
