摘    要:目的 探讨三维能量多普勒超声鉴别婴幼儿浅表血管瘤增生期与退化期的诊断价值。方法 根据Mulliken分类法与增殖细胞核抗原(Ki-67)表达,将经手术病理诊断为血管瘤的39例患儿分为增生期血管瘤及退化期血管瘤,比较不同分期血管瘤的血管指数(VI)、血流指数(FI)及血管血流指数(VFI)。结果 血管瘤增生期24例,VI 0.51±0.11,FI 38.06±6.64,VFI 19.54±5.52;退化期15例,VI 0.32±0.15,FI 30.81±10.29,VFI 11.07±6.47。增生期VI、FI、VFI均高于退化期,差异均有统计学意义(P均<0.05)。结论 三维能量多普勒超声能实时、清晰、动态显示肿块的血流情况,并对血管瘤分期的鉴别具有重要的临床意义。

Abstract:Objective To explore the value of three-dimensional power Doppler ultrasound in differential diagnosis of the proliferative and involuting stages of infantile superficial hemangioma. Methods According to Mulliken classification and PCNA (Ki-67) expression, 39 children with hemangioma confirmed by surgery and pathology were classified to proliferative hemangioma and involuting hemangioma, and vascular index (VI), blood flow index (FI) and vascular blood flow index (VFI) were analyzed and compared between the two stages. Results VI (0.51±0.11), FI (38.06±6.64) and VFI (19.54±5.52) of the proliferative stage were higher than those of the involuting stage (0.32±0.15, 30.81±10.29, 11.07±6.47, all P<0.05). Conclusion Three-dimensional power Doppler ultrasound could display the blood flow of tumors real-timely, clearly and quantificationally, therefore, which is valuable in differentiating stages of superficial infantile hemangioma.
