静脉注射微粒对家兔血浆TXA2、PGI2及肺动脉压力影响的研究

摘    要:目的研究输液微粒对家兔血浆TXA2和6-酮基-PGF1a及肺动脉压的影响.方法取24只家兔随机分为A、B、C三组,将三种含不同微粒数的溶液,即临床常用的静脉推注药液、同种药液经10 μm孔径滤过和经3μm孔径滤过的药液,分别注入三组家兔体内,并在推注前、推注后即刻、30 min、60 min、300 min五个时间点检测血浆TXA2和PGI2的代谢物TXB2和6-酮基-PGF1a的浓度,并进行肺动脉压监测,观察注射微粒对其影响.结果 TXB2的浓度随着5个不同的时间点时间推移均呈逐渐升高.其中A组在推注后60 min时的浓度与未推注前比较有显著意义(P<0.05),推注后300 min比较有明显差异(P<0.01);B组在推注后60 min和300 min的浓度与未推注前比较有显著意义(P<0.05).血浆6-酮基-PGF1a的浓度变化显示A组中第2、3两个时间点的浓度与未注时比较有显著意义(P<0.05).肺动脉压力变化呈现先高后低现象.结论为病人推注高浓度药物时,应用有效的终端过滤器进行截拦后推注较安全,对肺动脉高压与肺功能低下病人应慎用静脉推注给药.

Abstract:Objective To study the effect of intravenous injection of particles on thromboxane (TX) A 2 and prostaglandin(PG)I 2 level in plasma and pulmonary arterial pressure of rabbits.Methods 24 rabbits were randomly divided into group A, group B, and group C. Each group was injected with physic liquors containing different numbers of particles. Before the injection, physic liquors were filtrated by filters with 10um diameter and 3um diamater. TXA 2 and PGI2's metabolin-Thromboxane (TX) B 2 and 6-keto-prostacyclin F 1a before injection, 0 minutes, 30 minutes, 60 minutes and 300 minutes after injection.Results TXB 2 level increased with the time going on. Compared with before injection in group A and group B, TX B 2 level at 60 minutes and 300 minutes showed significant difference (P<0.05). (2) Compared with before injection in group A, 6-keto-prostacyclin F 1a at 0 minutes and 30 minutes after injection showed significant difference (P<0.01). (3) Pulmonary arterial pressure increased first and then reduced.Conclusion It's safer that high-level physic liquors are filtrated by effective terminal filters before intravenously injection. We should be cautious when injecting intravenously to patients with pulmonary hypertension or pulmonary dysfunction.
