PICC应用于造血干细胞移植的研究

摘    要:目的]对经外周静脉置入中心静脉导管(PICC)应用于造血干细胞移植病人中出现的问题进行分析并实施有效的护理对策.方法]采用美国生产的4Fr单腔导管,针对44例造血干细胞移植病人应用PICC出现的问题实施的护理经验进行总结.结果]PICC置管保留时间最短3 d,最长达1年2个月.其中感染1例,机械性静脉炎5例,导管异位1例,导管堵塞2例,导管尖端置管位置肿胀3例,静脉穿刺处渗血5例.结论]造血干细胞移植病人由于受免疫功能低下的影响,严格做好PICC导管的管理与护理才能延长病人的置管时间,以达到降低护士的工作量,提高病人生活质量的目的.

Abstract:Objective:To analyze problems existed in PICC applied for patients undergoing hematopoietic stem cells transplantation and to carry out effective nursing strategies for them.Methods:Both the experience of nursing care carried out with 4Fr single lumen of tube made in USA,and problems occurred in 44 cases of PICC patients undergoing hematopoietic stem cells transplantation were summarized.Results:The shortest time of catheter retaining among all cases were 3 days,and the longest was 14 months.Among all patients,only one case developed infection.5 cases developed mechanical phlebitis.Another case got catheter dystopia.Two cases developed catheter obstruction.3 cases developed swelling in catheter top location and 5 cases occurred errhysis in vein puncture area.Conclusion:Because of poor immune function,strict management and nursing care for PICC catheter can prolong the retaining time catheter for patients.So as to lower nurses' work quantity and promote the quality of life of patients.
