急性心肌梗死患者急诊与延迟冠状动脉介入治疗后左心功能变化的超声心动图观察

摘    要:目的应用超声心动图评价冠状动脉介入治疗(PCI)的临床疗效,比较急诊PCI与延迟PCI在改善急性心肌梗死(AMI)患者心功能中的价值。方法选择冠状动脉左前降支(LAD)为“罪犯血管”的AMI患者22例,急诊PCI组13例,延迟PCI组9例。应用双平面Simpson法测量患者术后不同时间段左室收缩功能及心腔内径的改变。结果术后1个月和3个月时两组患者心功能均较术后7d明显改善(P〈0.05),而左室内径较术后7d缩小(P〈0.05)。术后各阶段两组患者心功能比较差异无统计学意义。结论静脉溶栓后延迟PCI与急诊PCI术后1个月和3个月时均有心功能明显改善。

Abstract:Objective To evaluate the clinical effect of percutaneous coronary intervention(PCI) using routine echocardiography,and compare the ability of emergency PCI with delayed PCI in improving left ventricular function in patients with acute myocardial infarction(AMI).Methods Twenty-two AMI patients with left anterior descending(LAD) coronary to be the "criminal vessel" were divided into emergency PCI group and delayed PCI group.Biplane Simpson's formula was used to investigate the change of left ventricular systolic function in all patients.Results left ventricular systolic function improved significantly at 1 and 3 month in both group than 7days after PCI,while left ventricular diameter decreased significantly.No significant difference was found in left ventricular systolic function between the two groups at any follow-up time.Conclusion delayed PCI after initial antithrombotic agent infusion can improve left ventricular function as well as emergency PCI.Left ventricular function achieved significant improvement at 1 and 3 month's follow-up.
