超声引导下经皮穿刺治疗肝脓肿

摘    要:目的探讨肝脓肿介入治疗方法的优势及局限性。方法超声引导下经皮穿刺对35例肝脓肿进行脓汁引流、脓腔冲洗、注药治疗,并进行细菌培养、药敏感试验、抗感染治疗。结果35例全部穿刺成功,细菌培养阳性18例,阴性17例,除1例合并肿瘤,进行手术外,34例患者经超声介入治疗1~4周后达到临床治愈标准,治疗次数1—5次,平均治疗2.2次,未发生并发症。结论介入超声治疗肝脓肿创伤小、安全有效、无并发症,值得推广应用,但对于有合并症者,应严格掌握适应证。

Abstract:Objective To determine the advantage and limitation of percutaneous aspiration of hepatic abscess with ultrasound guidance.Methods Thirty-five patients with hepatic abscess were treated with percutaneous aspiration guided with ultrasound. And meanwhile, some cases were treated with antibiotic infusion,bacteria culture and drug sensitivity test were performed.Results All the patients were successfully punctured and the pathogens were observed in 18 cases, except 1 case complicating hepatic malignant tumor was treated with operation later.Conclusion The method of percutaneous aspiration of hepatic abscess with ultrasound guidance, which is safe, effective and has little complication, may play an important role in the treatment of hepatic abscess. While complicating other sort of disease, especially hepatic malignant tumor, the procedure should be carefully performed.
