摘    要:目的 探讨超声引导下胸腔内置管及超声随访监测肺底积液情况的临床应用价值.方法 将我院收治并确诊的120例中-少量结核性肺底积液患者,随机分为治疗组和对照组,各60例.治疗组患者在超声引导下行胸腔内置管术,进行胸腔积液的引流;对照组患者进行常规胸腔穿刺,比较两组患者穿刺成功率、胸腔粘连、引流量及并发症等情况.结果 本组120例患者,其中双侧积液37例,单侧积液83例;肺底积液位于右侧胸腔74例,位于左侧胸腔46例.治疗组穿刺成功率100%,对照组85%;治疗组胸水引流量(1200±86)ml,对照组(800±59)ml;治疗组发生胸膜粘连12例,对照组发生胸膜粘连21例;治疗组发生气胸3例,对照组发生气胸8例,两组比较差异有统计学意义(P﹤0.05).结论 超声引导下胸腔内置管引流术对中-少量结核性肺底积液治疗具有穿刺简单、引流彻底、疗效确切及并发症少等优点,超声随访监控具有实时、简便的特点,值得临床推广应用.

Abstract:Objects To explore the application value of ultrasound-guided intrathoracic drainage and real-time monitoring in treatment of subpulmonary effusion.Methods A total of 120 hospitalized patients with small to moderate tuberculo-subpulmonary effusion were randomized into two groups,including drainage treatment group(60 patients were treated with ultrasound-guided thorax catheterization to drain pleural effusion) and control group(60 patients were treated with conventional thoracentesis).Parameters such as successful thoracentesis rate,pleural adhesions,drainage volume and complications of the two groups were compared.Results In 120 patients,37 cases were bilateral effusion and 83 cases were unilateral effusion.Seventy-four cases of subpulmonary effusion located in the right side of the thorax and 46 cases located in the left side of the thorax.The success rates of puncture in treatment group and control group were 100% and 85%,respectively.Pleural effusion volume drained was(1200±86) ml in treatment group and(800±59) ml in control group.Pleural adhesion occurred in 12 cases of treatment group and 21 cases of control group.Pneumothorax occurred in 3 cases of treatment group and 8 cases of control group.There were significant differences between the two groups(P﹤0.05).Conclusion Ultrasound guided intrathoracic catheter drainage for the treatment of tuberculo-subpulmonary effusion is valuable because of its curative effect,easy puncture,complete drainage and few complications.Ultrasound follow-up and monitoring with the advantage of real-time and convenience may provide more helpful information.
