超声引导经皮穿刺输尿管镜上尿路取石术的临床应用

目的探讨B超引导经皮肾穿刺输尿管镜碎石治疗上尿路结石临床应用价值。方法B超引导经皮肾穿刺输尿管镜碎石治疗上尿路结石68例。结果68例中一期获得成功56例,l2例仍有残石,一次治愈率82.4%。B超引导穿刺均一次性成功,平均用时8 min,手术时间平均120 min。肾造瘘留置时间平均5 d,平均住院时间8 d,术中出血少。结论B超引导经皮肾穿刺输尿管镜碎石具有定位准确、方便、损伤小、结石清除率高、恢复快、住院时间短、并发症少等优点,值得临床推广应用。

Chimioal application value of percutaneous nephrolithotomy und er ultrasound guidance.

Objective To evaluate the feasibility and availability of minimally invasive percutaneous nephrolithotomy(MPCNL) under ultrasound guidance for treatment of calculus of upper urinary tract.Methods 68 patients with calculus of upper urinary tract were investigated after treated with minimally invasive percutaneous nephrolithotomy under ultrasound guidance.Results In all,56 cases were cured successfully,residual calculi were confirmed by X ray or ultrasound postoperatively in 12 cases,the stone-free rate was 82.4%.All the percutaneous punctures under ultrasound guidance were successful onetimely,the average time of percutaneous puncture was 8 minutes and the average operation time was 120 minutes.The average time of having keeped nephrotomic catheter was 5 days.the average hospitalization was 8 days.Blood loss was less,no major complications were noted.Conclusion Minimally invasive percutaneous nephrolithotomy under ultrasound guidance is effective and safe for treatment of calculus of upper urinary tract.
