结直肠癌患者癌组织CTEN、E-cadherin及Vimentin蛋白表达水平的意义探讨

摘    要:目的研究结直肠癌(CRC)患者癌组织内羧基末端张力蛋白样分子(CTEN)、上皮型钙黏素(E-cadherin)及波形蛋白(Vimentin)的表达水平及意义。方法选取回顾性分析方法,研究对象为2017年1月至2019年12月湖南中医药高等专科学校附属第一医院收治的90例CRC患者。所有患者均经CRC手术切除获取癌组织、癌旁组织标本。比较癌组织、癌旁组织中CTEN、E-cadherin、Vimentin的阳性表达率,分析CRC临床病理参数与CTEN、E-cadherin、Vimentin表达之间的关系,并分析CTEN、E-cadherin、Vimentin的相关性。结果与癌旁组织相比,癌组织中CTEN、Vimentin的阳性表达率更高,E-cadherin的阳性表达率更低(P<0.05)。TNM分期为T1~3期CRC患者的癌组织CTEN、Vimentin阳性表达率较T4期CRC患者更低,且E-cadherin阳性表达率更高(P<0.05);伴发淋巴结转移及神经侵犯的癌组织较未出现淋巴结转移及神经侵犯的癌组织,CTEN、Vimentin阳性表达率更高,且E-cadherin阳性表达率更低(P<0.05);肿瘤直径<5 cm的癌组织较肿瘤直径≥5 cm的癌组织,CTEN、Vimentin阳性表达率更低(P<0.05);肿瘤分化程度低的癌组织较肿瘤分化程度高的癌组织Vimentin阳性表达率更低(P<0.05)。癌组织中E-cadherin表达水平与CTEN、Vimentin呈显著负相关(r=-0.338、-0.188,P=0.017、0.003);癌组织中Vimentin表达水平与CTEN呈显著正相关(r=0.389,P<0.001)。结论CTEN、E-cadherin及Vimentin阳性表达率与CRC患者的淋巴转移、神经侵犯、TNM分期密切相关,CTEN、Vimentin阳性率与肿瘤大小密切相关,Vimentin阳性率与CRC分化程度密切相关。

Abstract:Objective To study the expression levels and significance of carboxy-terminal tensin-like molecules(CTEN),epithelial cadherin(E-cadherin) and Vimentin in cancer tissues of patients with colorectal cancer. Methods The retrospective analysis method was selected,and 90 patients with colorectal cancer admitted to the First Affiliated Hospital of Hunan College of Traditional Chinese Medicine from January 2017 to December 2019 were selected as the research objects. All patients were resected by CRC to obtain specimens of cancerous and paracancerous tissues;compare cancerous and paracancerous tissues The positive expression rate of CTEN,E-cadherin,and Vimentin in CRC,analyze the relationship between CRC clinicopathological parameters and the expression of CTEN,E-cadherin,and Vimentin,and analyze the correlation of CTEN,E-cadherin,and Vimentin. Results Compared with adjacent normal tissues,the positive expression rates of CTEN and Vimentin in cancer tissues were higher,while the positive expression rates of E-cadherin were lower(P < 0. 05). The positive expression rates of CTEN and Vimentin in cancer tissues of patients with TNM stage T1-3 colorectal cancer were lower than those of patients with T4 colorectal cancer,and the positive expression rates of E-cadherin were higher(P < 0. 05). The positive expression rates of CTEN and Vimentin were higher in cancer tissues with lymph node metastasis and nerve invasion than those without lymph node metastasis and nerve invasion,and the positive expression rates of E-cadherin were lower(P < 0. 05). The positive expression rates of CTEN and Vimentin in tumor tissues with tumor diameter < 5 cm were lower than those with tumor diameter ≥5 cm(P < 0. 05). The Vimentin positive expression rate was lower in cancer tissues with low tumor differentiation than in cancer tissues with high tumor differentiation(P < 0. 05). The expression level of E-cadherin in cancer tissues was significantly negatively correlated with CTEN and Vimentin(r =-0. 338,-0. 188,P = 0. 017,0. 003). The expression level of Vimentin in cancer tissues was positively correlated with CTEN(r = 0. 389,P < 0. 001). Conclusion The positive expression rates of CTEN,E-cadherin and Vimentin were closely correlated with lymph node metastasis,nerve invasion and TNM staging in patients with colorectal cancer;the positive rates of CTEN and Vimentin were closely correlated with tumor size;the positive rates of Vimentin and Vimentin were closely correlated with the differentiation degree of colorectal cancer;the expressions of CTEN,E-cadherin and Vimentin were correlated to some extent.
