阿司匹林合用两种不同剂量氯吡格雷治疗不稳定型心绞痛疗效对比

目的对比观察阿司匹林合用两种不同剂量氯吡格雷治疗不稳定型心绞痛的临床疗效及安全性。方法将不稳定型心绞痛患者113例随机分为对照组和观察组，两组病患均行常规药物治疗基础之上合用阿司匹林和氯吡格雷，疗程均为6周。对照组57例氯吡格雷服用方法为75mg／d，1次／d；观察组56例氯吡格雷服用方法为50mg／d，1次／d。观察并分析两组总有效率、心绞痛每日发作频率、持续时间、不良心脏事件发生率和不良反应发生情况。结果对照组总有效率为98．25％，观察组为96．43％，两组无显著性差异（P〉0．05）；两组心绞痛每周发作频率、持续时间和不良心脏事件发生率比较，差异无统计学意义（P〉0．05）；对照组发生轻微不良反应4例，观察组2例。结论在常规药物治疗基础上，阿司匹林合用50mg／d氯吡格雷治疗不稳定型心绞痛疗效好且安全性高，值得临床推广应用。

A comparative study on efficacy of aspirin combined with two different dosages of clopidogrel in treatment of unstable angina.

Objective To comparative explore the clinical efficacy and safety of aspirin in combination with two different dosages of clopi- dogrel in treatment of unstable angina. Methods A total of 113 patients with unstable angina in this hospital were randomly divided into control group and observation group, all patients of these two groups were administrated with combination of aspirin and clopidogrel on the basis of conven- tional drug treatment, and the course of treatment was 6 weeks. The dosage of clopidogrel was 75 rag/d, qd in control group ; and it was 50 mg/ d, qd in observation group. The total effective rate, daily frequency of angina seizure, duration and incidence of adverse cardiac events and ad- verse events were compared and analyzed. Results The total efficacy rate was 98.25% in control group, and it was 96.43% in observation group, there was no significant difference （ P 〉 O. 05 ）. In comparison with control group, the total effective rate, frequency and duration of angi- na attack and adverse cardiac events in observation group, the difference was not statistically significant （ P 〉 0.05 ） . There were four cases and two cases of minor adverse reactions occurred in control group and observation group respectively. Conclusion The combination of aspirin and clopidogrel （50 mg/d） based on conventional drug treatment is effective and safe in treatment of patients with unstable angina. This regimen for treatment of patients with unstable angina is worthy to be recommended for further study.
