下腹正中切口腹膜前修补术治疗复发疝32例临床分析

目的探讨下腹部正中切口腹膜前修补术治疗腹股沟复发疝的安全性和有效性。方法 32例腹股沟复发疝行下腹正中切口腹膜前修补术,观察手术效果和术后并发症发生情况,随访观察有无复发。结果本组手术过程顺利,单侧平均手术时间（38.0±5.1）min,双侧平均手术时间（55.0±6.2）min,手术出血量4-9mL,平均手术出血量（5.0±0.8）mL;术后疼痛轻微,未使用镇痛药物;术后无切口感染、阴囊积液、缺血性睾丸炎、术区异物感、慢性疼痛等并发症发生;术后1-2d下床活动,住院时间2-5d,平均3.4d,随访8-24个月无复发。结论下腹正中切口开放式腹膜前修补术治疗复发疝操作简单,疗效确切。

Clinical analysis of preperitoneal repair via inferior abdominal median incision for recurrent inguinal hernia in 32 cases

Objective To evaluate the safety and effect of preperitoneal repair via inferior abdominal median incision on recurrent inguinal hernia. Methods Thirty-two cases of recurrent inguinal hernia underwent preperitoneal repair via inferior abdominal median incision, and were observed the surgical outcome, postoperative complications and recurrence. Results Operation was successfully completed in all patients. The mean operation lasting time of unilateral recurrent inguinal hernia was （38.0±5.1） minutes, and the mean operation lasting time of bilateral recurrent inguinal hernia was （55.0±6.2） minutes. The blood loss was 4 to 9 mL, averagely （5.0±0.8） mL. Postperative pain was slight, without using analgesic drugs. No complications were found, such as postoperative wound infection, scrotal hydrocele, ischemic orchitis, the surgical foreign body sensation and chronic pain. In 1 to 2 days after operation, patients began off-bed activity. The hospitalization duration was 2 to 5 days, averagely 3.4 days. All patients were followed up for 8 to 24 months, with no recurrence. Conchlsions Preperitoneal repair via inferior abdominal median incision is simple and effective for recurrent hernia.
