子宫切除患者真实体验质性研究的Meta整合

目的：系统评价子宫切除后女性的自我概念,真实感受、态度和情感体验,为医疗保健者向妇女提供子宫切除术综合信息,为其提供更优质的护理照护和关怀。方法：计算机检索PubMed、Embase、CINAHL、Cochrane Library、Web of Science、Sino Med、中国知网、维普、万方8个数据库,收集子宫切除患者真实生活感受与情感体验的质性研究,检索时间为建库至2021年11月10日。采用澳大利亚 JBI 循证卫生保健中心质性研究质量评价标准对文献进行评价,并采用汇集性整合方法对结果进行整合。结果：共纳入文献13篇,提炼出47个完整的结果,将相似结果归类为8个类别,得到3个整合结果：术前决策的心路历程;术后自我概念的变化;术后长期经历及应对策略。结论：子宫切除术对患者的身心健康产生深远而持久的影响,护理工作者应关注子宫切除女性的情感需求和心理变化,提供支持和帮助,促进患者适应无子宫生活,提高身心健康和生活质量。

Qualitative researches on real experience of hysterectomy patients: a meta synthesis

Objective: To systematically evaluate the self-concept, true feelings, attitudes and emotional experience of women after hysterectomy, so as to help medical and health care providers provide women with comprehensive information of hysterectomy and provide them with better nursing care. Methods: Totally eight databases including PubMed, EMbase, CINAHL, Cochrane Library, Web of Science, Sino Med, CNKI, VIP and Wanfang were searched by computer to collect the qualitative research on the real life and emotional experience of patients with hysterectomy. The retrieval time was from the establishment of the database to November 10, 2021. The quality evaluation standard of qualitative research of JBI evidence-based health care center in Australia was used to evaluate the literature, and the results were integrated by the method of pooled integration. Results: A total of 13 literatures were included, 47 complete results were extracted, and the similar results were classified into 8 categories. Three integrated results were obtained: the mental process of preoperative decision-making; Changes of self-concept after operation; Long term postoperative experience and coping strategies. Conclusion: Hysterectomy has periodic impact on patients" physical and mental health. Nursing workers should pay attention to the emotional needs and psychological changes of women after hysterectomy, provide support and help, promote patients to adapt to the life without uterus, and improve their physical and mental health and quality of life.
