宫腔镜电切术治疗子宫内良性病变121例疗效分析

目的:探讨宫腔镜电切术治疗子宫内良性病变的疗效。方法:应用连续灌流式宫腔镜,对121例子宫内良性病变进行宫腔镜电切手术治疗,其中子宫粘膜下肌瘤20例,功能失调性子宫出血30例,子宫内膜息肉15例,宫颈肌瘤25例,宫颈宫腔复合性病变20例,过期流产4例,宫内残留7例。结果:120例手术一次成功,1例过期流产二次手术成功,无子宫穿孔、出血及肠管损伤,3例轻度水中毒。宫腔镜电切手术时间为8~97min,平均25min,出血量5~150ml,平均45ml。患者的临床症状都得到改善,治疗有效率为100%。结论:宫腔镜手术无切口,手术时间短,创伤小,出血量少,术后恢复快。故在有条件的医院,应将宫腔镜手术作为治疗宫内良性病变的首选方法。

Clinical Anallysis on Treatment of 121 Patients with Intrauterine Benign Lesion by Hysteroscopic Electroresection

Objective: To assess the clinical effect of hysteroscopic electroresection in the treatment of intrauterine benign le-sion.Methods: One hundred and twenty-one women with intrauterine benign lesion received hysteroscopic transcervical resection with a continuous flow hysteroscope.They consisted 20 uterine submucous myoma,30 dysfunctional uterine bleeding,15 en-dometrial polyp,25 cervical myoma,20 cervicouterine tumor,4 missed abortion and 7 residual embryo.Results: One-hun-dred twenty cases were sucessful from taking taicrgconly one operation,one case of missed abortion was excised with two opera-tions,and no uterine perforation,hemorrhage,or intestinal injury,but 3 cases had slight water toxicity.Operation duration was 8~97 minutes(average time 25 min),and the blood lose 5~150ml(average quantity 45ml).The clinical symptoms were reliv-ered significantly in all cases,and the healing rate was 100%.Conclusions: The transcervical resection has no incision wound,shorter of operating time,mini-invasion,less bleeding and faster recovery after operation.Therefore,transcervical resection surgery had to be the first choice of treatment for benign intrauterine lesions.
