心理干预对介入治疗心脏病患者心理状态的影响

目的探讨介入治疗心脏病患者术前的心理状况及心理干预对其影响。方法将择期行经皮穿刺冠状动脉扩张成形术 (PTCA)加支架植入术和永久起搏器植入的患者 ,随机分为对照组和观察组。术前两组进行常规术前治疗、护理 ,对观察组进行系统的心理护理干预。应用症状自评量表 (SCL 90 )对术前和术后 3～ 6个月时的心理状态进行调查。结果两组SCL 90评分均显著高于正常组 (P <0 0 5 ) ,两组接受介入治疗后SCL 90评分均显著降低 (P <0 0 0 1 ) ,术后观察组评分明显低于对照组 (P <0 0 0 1 )。结论 (1 )接受介入治疗的心脏病患者术前和术后均有不同程度的心理问题 ;(2 )介入治疗可改善心脏病患者的心理状态 ;(3)术前心理护理干预可改善接受介入治疗患者的心理状态。

Effect on the psychological status for patients with heart disease treated by intervened therapy through psychological intervention

Objective To study the psychological status for patients with heart disease before intervened therapy, and the effect after psychological intervention. Method Patients with heart disease going to treated by PTCA plus bracket insertion and permanent pacemaker were selected and divided into control and observation group, pre-operative routine nursing and treatment were given for the two group, additional systematic psychological nursing was given for observation group, then SCL 90 were used for psychological status survey before and 3 6 months after operation. Results The SCL 90 scoring for the two groups were significant higher than normal group ( P <0.05). The SCL 90 scoring for the two groups were significant reduced after treated by intervened therapy ( P <0.01). The SCL 90 scoring for the observation groups after operation was significant lower than control group ( P <0.001). Conclusion Some psychological problems were existed among patients with heart disease before and after having intervened therapy. Intervened therapy will improve the patients' psychological status, and pre-operation psychological intervention will improve the patients psychological status before intervened therapy.
