流行性腮腺炎患者T淋巴细胞亚群的检测分析

目的 分析不同年龄段流行性腮腺炎患者T淋巴细胞亚群的变化情况,以探讨其在腮腺炎病毒感染情况下细胞免疫功能的变化.方法 收集2012年2月～2013年5月西安市第八医院收治的流行性腮腺炎患者共97例（其中男性68例,女性29例;年龄2～44岁;7岁以下年龄组21例,7～12岁年龄组35例,12岁以上年龄组41例）.用流式细胞术检测治疗前患者及95例健康对照者（7岁以下年龄组38例,7～12岁年龄组31例,12岁以上年龄组26例）外周血CD3＋,CD3＋ CD4＋和CD3＋ CD8＋T淋巴细胞亚群绝对数量的变化,计算CD3＋ CD4＋/CD3＋ CD8＋比值,并对不同年龄段患者的检测结果进行了分析.结果 7岁以下患者组CD3＋,CD3＋ CD4＋,CD3＋ CD8＋ T淋巴细胞亚群绝对计数和CD3＋ CD4＋/CD3＋ CD8＋分别为2 272±1106个/μl,953±440个/μl,1 191±683个/μl和0.87±0.25;对照组分别为2 035±715个/μl,1 260±499个/μl,723±281个/μl和1.85±0.71.7～12岁患者组分别为1 642±768个/μl,674±343个/μl,885±454个/μl和0.81±0.30;对照组分别为1 627±633个/μl,903±371个/μl,674±269个/μl和1.36±0.29.12岁以上患者组分别为1 528±510个/μl,630±225个/μl,833±309个/μl和0.79±0.24,对照组分别为1 403±432个/μl,762±223个/μl,592±199个/μl和1.31±0.23.各年龄组流行性腮腺炎患者与其对照组比较,CD3＋ T细胞数量变化差异无统计学意义（t=0.086～1.037,P＞0.05）;CD3＋ CD4＋ T细胞数量降低,CD3＋ CD8＋ T细胞数量升高,差异有统计学意义（t=2.329～3.876,P＜0.05）;CD3＋ CD4＋ /CD3 CD8比值明显低于其正常对照组,差异有统计学意义（t=6.164～8.758,P＜0.001）.结论 腮腺炎病毒感染患者T淋巴细胞亚群数量明显改变,可能引起患者抗病毒免疫功能的紊乱.

Significance of Detecting T Lymphocyte Subsets in Patients with Epidemic Parotitis

Objective Epidemic parotitis （mumps） is caused by the mumps virus,occurred especially in children and adolescents.Due to viral infectious disease is usually relative with abnormal number and function of immune cells,so investigated the significance of T lymphocyte subsets in patients with mumps,and try to explore the changes of cell-mediated immunity in these patients.Methods 97 patients with mumps and 95 healthy controls were enrolled in this study and divided into 3 groups according to the years of age （＜7,7～12 and ＞12 years old）.Absolute counts of CD3＋,CD3＋ CD4＋ and CD3＋CD8＋ T lymphocytes in peripheral blood of the subjects were analyzed by flow cytometry,and CD3 CD4 /CD3 CD8 ratio were calculated by the software.Results CD3,CD3 CD4,CD3 CD8 T lymphocyte subsets counts and CD3 ＋CD4/ CD3 CD8 ratio were showed as below respectively：①＜7 years old group：2 272±1 106 cells/μl,953±440 cells/μl,1191 ±683 cells/μl and 0.87 ± 0.25 in patients,2 035±715 cells/μl,1 260±499 cells/A,723 ± 281 cells/μl and 1.85±0.71 in its controls ; ② 7 ～ 12 years old group：1 642 ± 768 cells/μl,674 ± 343 cells/μl,885 ± 454 cells/μl and 0.81 ± 0.30 in patients,1 627±633 cells/μl,903±371 cells/μl,674±26 cells/μl and 1.36±0.29 in its controls;③＞12 years old group：1 528±510 cells/μl,630±225 cells/μl,833±309 cells/μl and 0.79±0.24 in patients,1 528±510 cells/A,762±223 cells/μl,592±199 cells/μl and 1.31±0.23 in controls.Compared with controls,the counts of CD3 T lymphocytes were no statistical differences in all of the patients （t=0.086～1.037,P＞0.05）.CD3 ＋CD4 ＋T lymphocyte numbers and CD3＋ CD4＋ /CD3＋ CD8＋ ratio were significantly lower in all of the mumps patients than that of the controls （t=2.334～ 2.606,P＜0.05 and t=6.164～8.758,P＜0.001 respectively）,while the counts of CD3 CD8 T cells was significantly increased in all patient groups compared to the control groups with statistical differences （t=2.329～3.876,P＜0.05）.Conclu
